RENOVATE SPACE FOR HEAMATOLOGY/ONCOLOGY AND CHEMOTHERAPY TREATMENT CENTER, BLDG 77, JAMES H. QUILLEN VA MEDICAL CENTER, MOUNTAIN HOME, TN PROJECT No. 621-14-106 VA249-14-R-0186 Phase I Estimated Solicitation Date: 27 December 2013 Phase I Estimated Response Date: 27 January 20124 Estimated Award Date: 28 April 2014 The Department of Veterans Affairs, Veterans Administration Medical Center is preparing to issue a solicitation (Request For Proposals-RFP) for a two-phase design-build contract to renovate approximately 10,000 square feet of existing space on the first floor of Building 77 at the James H. Quillen VA Medical Center into a Hematology/Oncology Referral Center in accordance with VA Design and Space Planning Criteria and VA Master Specifications. The intention of the project is to expand the existing Chemotherapy space into the adjacent waiting area and extended care by providing the functions listed above. The reception area and waiting area (Located in J108A and J108 respectively) will be renovated. The exam rooms located in J109, J112, J113, J114 and J115 will be rebuilt in the demolished waiting area space. The reception desk will be renovated to serve as a centralized check in station for four clerks. SET-ASIDE: 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) NAICS: 541310 Architect Design Services SMALL BUSINESS SIZE STANDARD: $33.5 million PROJECT MAGNITUDE: $500,000 to $1,000,000. PROJECT LOCATION: James H. Quillen VA Medical Center, Mountain Home, TN PROJECT OVERVIEW: The project will require the selected contractor to provide all technical design and construction services, supervision, labor, materials, tools and equipment necessary for a complete engineering and construction project to renovate into spaces as described above for use in implementation of initiative. The Two-Phase Design-Build solicitation and evaluation process shall be conducted in accordance with Federal Acquisition Regulation (FAR) Subpart 36.3 (Two-Phase Design-Build Selection Procedures). The issued RFP shall set forth the requirements for responding to Phase I of the solicitation, as well as all technical evaluation factors and sub-factors for both Phase I and Phase II. At the closing date of the Phase I Solicitation, the government shall evaluate responsive proposals received in response to Phase I of the solicitation to short-list no more than three (3) contractors to Phase II. Only those successful firms selected as a result of Phase I evaluations will be requested to submit technical and pricing proposals for Phase II. Phase II proposals shall be evaluated in accordance with Phase II Evaluation Process.
