Description
This is a combined synopsis/solicitation issued in accordance with FAR 12.603. The U.S. Department of Health and Human Services, Office of Medicare Hearings and Appeals (OMHA) is looking to lease secured parking spaces at or near their office space in Miami, Florida that meets all of the following criteria: Up to 28 parking cards needed per month; Guaranteed number of parking spaces corresponds to number of parking cards issued that month; Flexible Pricing per parking card based on need per month with a minimum of 30-day notice required; The parking must be accessible to mobility impaired employees and visitors; Preference for parking in the building is for the safety of the Judges, Hearing Office Director, and handicapped visitors as well as employees that may need special services to return to work, like returning from surgery, or disabilities; Security cameras at each entrance and exit of the garage; AND Hired security personnel to provide security services throughout the garage. Location: Due to security and safety of OMHA personnel, Judges, Hearing Office Director, and visitors, parking facility must be no more than 1 block from the OMHA office located at 100 SE 2nd Street, Miami, FL 33131 Hours of Operation: The parking facility must be accessible 24 hours per day, including weekends. Period of Performance: 12 month contract to be billed monthly. Anticipated Contract Type: Firm Fixed Price. Provisions and Clauses: The Provision FAR 52.212-1 Instructions to Offerors-Commercial Items (JUL 2013) applies to this acquisition. The Provision FAR 52.212-2 is not applicable. Offerors are to include a completed copy of the Provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (NOV 2013) with this offer. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via http://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Feb 2012), applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (May 2012), applies to this acquisition. The Government will award a purchase order resulting from this synopsis/solicitation to the responsible lowest priced offeror that meets all of the technical requirements.
