- Proficiency Testing Analytes; Statement of Work-Proficiency Testing-Analytes 1. This is a COMBINED SYNOPSIS/SOLICITATION for commercial services prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 2. Solicitation number RFQ-14-PHX-02 is being issued as a Request for Quote (RFQ). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-71. The full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation Supplement (HHSAR) can be accessed via the Internet at http://farsite.hill.af.mil/vffara.htm (FAR) and http://farsite.hill.af.mil/vfhhsara.htm (HHSAR). 4. This requirement is being solicited as a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 325413 - In-Vitro diagnostic Substance Manufacturing, and the applicable size standard is 500 employees. 5. This requirement consists of 5 line items: 0001 Proficiency Testing-Analytes, POP 1/1/14 - 12/31/14, see attached listing of testing. 0002 Proficiency Testing-Analytes, POP 1/1/15 - 12/31/15, see attached listing of testing. 0003 Proficiency Testing-Analytes, POP 1/1/16 - 12/31/16, see attached listing of testing. 0004 Proficiency Testing-Analytes, POP 1/1/17 - 12/31/17, see attached listing of testing. 0005 Proficiency Testing-Analytes, POP 1/1/18 - 12/31/18, see attached listing of testing. 6. The Statement of Work and specifications are attached to this notification. The requirement is for Proficiency Testing Analytes. 7. The location of contract delivery is Hopi Health Care Center, Highway 264, Mile Marker 388, Polacca, Arizona 86042, and the point of delivery is FOB Destination to the same address. (a) The Government may extend the term of this contract by written notice to the Contractor within 10 calendar days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years. 52.232-19 Availability of Funds for the Next Fiscal Year (APR 1984) Funds are not presently available for performance under this contract beyond 12/27/14. The Government's obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise for performance under this contract beyond 12/27/14, until funds are made available to the Contracting Officer for performance and until the Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer. 10. The Defense Priorities and Allocations System (DPAS) is not Applicable to this acquisition. 11. RESPONSES ARE DUE BY: Tuesday, January 7, 2014 by 11:00 a.m. Arizona Standard Time (NOTE Arizona does not observe Daylight Savings Time).
