- Additional Terms and Conditions (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. (ii) Solicitation number IHS-14-HQ-RFQ-00006 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-71. (iv) The North American Industry Classification System (NAICS) code is 541611 with a small business size standard of $14M. This acquisition is set-asided for small businesses only. (v) Fill out the attached, Enclosure 1, Schedule of Prices for all Contract Line Items. Period of Performance: one year base period and 2 one year option periods. (vi) The Indian Health Service (IHS), Improving Patient Care (IPC) Program, has a need for services to include training and technical assistance for new National IPC staff as well as approximately 40-50 teams from Indian Health Service, Tribal and Urban (I/T/U) facilities that will participate in the final collaborative - IPC 5. Training and technical assistance are also needed for approximately 90 sites that will be participating in the Quality and Innovation Learning Network (QILN). See the attached PWS for additional details. (vii) The delivery and acceptance term is FOB Destination. (viii) FAR 52.212-1 - Instructions to Offerors - Commercial Items (Jul 2013): Quotes shall be submitted on company letterhead stationery, signed and dated; it shall include: (1) The solicitation number; (2) The name, address, and telephone number of the offeror; (3) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. See the attached, Enclosure 5, Evaluation Factors for Award; (4) Price and any discount terms; (5) "Remit to" address, if different than mailing address; (6) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); (7) Acknowledgment of Solicitation Amendments. (ix) 52.212-2, Evaluation - Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Questions regarding this RFQ and any terms and conditions must be submitted in writing by January 3, 2014, 4:00 pm EST. (xvi) Interested vendors capable of furnishing the government with the item specified in this combined synopsis/solicitation should submit their quotation to the below email address.
