- Part 12 (Combined Synopsis) - Federal Business Opportunities Posting Location: Quentin N. Burdick Memorial Healthcare Facility in Belcourt, ND Title: Preventive Maintenance Service agreement at Quentin N. Burdick Memorial Healthcare Facility, in Belcourt, North Dakota (RFQ-14-016) Description(s): Preventive Maintenance Service agreement for washer/sterilizer/surgery table equipment in the OR department at Belcourt Healthcare facility. THIS SOLICITATION IS ISSUED AS AN UNRESTRICTED OPEN MARKET WITH NO SET A SIDE. Preventive Maintenance Service agreement at Quentin N. Burdick Memorial Healthcare Facility, in Belcourt, North Dakota. Period of Performance will be January 1, 2014 to December 31, 2014. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994) (Public Law 103-355) & FAR 37.4, Non-personal Health Care Services Contract (10 U.S.C. 2304 and 41 U.S.C.253). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Aberdeen Area Indian Health Service (IHS) intends to award a firm fixed-price, non-personal service, purchase order, commercial item, in response to Request for Quote (RFQ) 14-016. This solicitation is issued as an Open Market with no Set a Side. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-71. The associated North American Industry Classification System code is 811219 and size standard is $19.0 million. Period of performance: This contract will be from January 1, 2014 through December 31, 2014. Location is at the Quentin N. Burdick Memorial Healthcare Facility in Belcourt, ND 58316. Basis for award shall be based on the Lowest Price Technically Acceptable "LPTA" bid package. The unit pricing must be an all inclusive cost (to include but not be limited to transportation, food, lodging, per diem, federal, state and local taxes), plus all other costs pertinent to the performance of this contract. Utilize your most competitive and reasonable rates, 12 Months @ a rate of $______________ for a yearly total of $______________. Attached are the Federal Acquisition Regulations (FAR) and the Health & Human Services Acquisition Regulation (HHSAR) clauses that are applicable including the following: FAR 52.212-1 - Instructions to Offerors - Commercial Items (July 2013): Quotes shall be submitted on company letterhead stationery, signed and dated; it shall include: 1. Solicitation number RFQ-14-016 2. Closing date of January 3, 2014 at 3:00pm CST. 3. Name, address, telephone number of the offeror and email address of the contact person. 4. Technical description of the item/service being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary. 5. Terms of any express warranty 6. Price and any discount terms. 7. "Remit to" address, if different than mailing address. 8. A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically 9. Acknowledgement of Solicitation Amendments. 10. Meet the requirements of the Statement of Work 11. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. 12. An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically via http://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision.
