Attachment 3a DOL Wage Determination Attachment 2 Offerors Schedule Attachment 4c Sample Statement of Work Attachment 4b Sample Bid Schedule Attachment 1 Statement of Work Attachment 5 BPA terms and Conditions Synopsitation Attachment 4a Sample Agenda Attachment 3b DOL Wage Determination This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, supplemented with additional information included in this notice. This announcement constitutes one (1) solicitation which will result in at least two (2) Firm Fixed Price Blanket Purchase Agreement (BPA) awards with at least one BPA in each of the following locations: The areas within 50 miles of the following cities; Charleston, SC; Charlotte, NC; Williamsburg, VA or 75 miles from Myrtle Beach, SC. Anticipated awards will be the result of Small Business set-aside. However, the Government reserves the right to award more or less than the anticipated BPAs listed above. Proposals are being requested and written solicitations will not be issued. Solicitation Number FA441-14-R-0005 is hereby issued as a Request for Proposal. This combined synopsis/solicitation and incorporated provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-71, effective 25 Nov 2013. The North American Industry Classification System (NAICS) code for this project is721110 and Standard Industrial Classification (SIC) code X1AB. The Size Standard for NAICS 721110 is $30M. This acquisition is solicited as a 100% set-aside for Small Business Concerns The contractor shall provide all labor, equipment, materials, tools, parts, shipping, and all other items necessary to provide provide 4-star quality lodging that possesses necessary facilities to host Special Tactics Reintegration & Family Sustainment Program events in accordance with the attached Statement of Work. The lodging facility shall be within 50 miles of the following cities; Charleston, SC; Charlotte, NC; Williamsburg, VA or 75 miles from Myrtle Beach, SC. Offerors shall submit pricing for CLINS 0001 to 0006 for "peak season" and CLIN 0007 through 0012 "off season" timeframes. Peak season and off season will be defined by the contractor using the "Offer Schedule". These events are comprised of various commodities and services that may or may not be required for individual events, refer to the Statement of Work for a complete description of required services for each CLIN. Please provide pricing for the following base period year. Initial ordering period will be (Estimated 1 Mar 2014 - 28 Feb 2015). The total duration of the BPA will not exceed 5 years, 1 Mar 2014 through 28 Feb 2019. (Note) The BPA pricing schedule will be renewed prior to entering any subsequent year of performance. Please include ammenities such as: snacks, set up fees, parking, audio visual for meeting space (Off season pricing) Note (*Please enter the pricing information on the "Offer Schedule" attachment 3 of the solicitation*) Workload Estimates: 5 year estimate of use is 10 events or approximately 2 event per year. Each event could include up to 50 adults and 40 children and 35 rooms. Delivery Terms are FOB Origin for offerors location in accordance with the attached Statement of Work. The following provisions and clauses apply to this solicitation and will remain in full force in any resultant award: 52.204-7 -- System for Award Management. As prescribed in 4.1105(a)(1), use the following provision: System for Award Management (Jul 2013) (a) Definitions. As used in this provision- "Data Universal Numbering System (DUNS) number" means the 9-digit number assigned by Dun and Bradstreet, Inc. (D&B) to identify unique business entities. "Data Universal Numbering System+4 (DUNS+4) number" means the DUNS number means the number assigned by D&B plus a 4-character suffix that may be assigned by a business concern. (D&B has no affiliation with this 4-character suffix.) This 4-character suffix may be assigned at the discretion of the business concern to establish additional System for Award Management records for identifying alternative Electronic Funds Transfer (EFT) accounts (see the FAR at Subpart 32.11) for the same concern. "Registered in the System for Award Management (SAM) database" means that- (1) The Offeror has entered all mandatory information, including the DUNS number or the DUNS+4 number, the Contractor and Government Entity (CAGE) code, as well as data required by the Federal Funding Accountability and Transparency Act of 2006 (see Subpart 4.14), into the SAM database; and (2) The offeror has completed the Core, Assertions, and Representations and Certification, and Points of contact sections of the registration in the SAM database; (3) The Government has validated all mandatory data fields, to include validation of the Taxpayer Identification Number (TIN) with the Internal Revenue Service (IRS). The Offeror will be required to provide consent for TIN validation to the Government as a part of the SAM registration process. (4) The Government has marked the record "Active". (b) (1) By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. (2) The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation "DUNS" or "DUNS+4" followed by the DUNS or DUNS+4 number that identifies the offeror's name and address exactly as stated in the offer. The DUNS number will be used by the Contracting Officer to verify that the offeror is registered in the SAM database. (c) If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. (1) An offeror may obtain a DUNS number- (i) Via the internet at http://fedgov.dnb.com/webform or if the offeror does not have internet access, it may call Dun and Bradstreet at 1-866-705-5711 if located within the United States; or (ii) If located outside the United States, by contacting the local Dun and Bradstreet office. The offeror should indicate that it is an offeror for a U.S. Government contract when contacting the local Dun and Bradstreet office. (2) The offeror should be prepared to provide the following information: (i) Company legal business name. (ii) Tradestyle, doing business, or other name by which your entity is commonly recognized. (iii) Company physical street address, city, state and Zip Code. (iv) Company mailing address, city, state and Zip Code (if separate from physical). (v) Company telephone number. (vi) Date the company was started. (vii) Number of employees at your location. (viii) Chief executive officer/key manager. (ix) Line of business (industry). (x) Company Headquarters name and address (reporting relationship within your entity). (d) If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. (e) Processing time, which normally takes 48 hours, should be taken into consideration when registering. Offerors who are not registered should consider applying for registration immediately upon receipt of this solicitation. (f) Offerors may obtain information on registration at https://www.acquisition.gov. (End of Provision) FAR 52.212-1, Instructions to Offerors - Commercial Items (Jul 2013) is amended to read: Offeror shall submit signed and dated offer to 24 SOW/PK, Attn: Michael Wenrick, 223 Cody Avenue, Hurlburt Field FL 32544, by 16 Jan 2014, 2:00 p.m. CST. Submit signed and dated Offer Schedule marked with solicitation number FA441-14-R-0005. The offeror may submit an electronic copy of their signed and dated offer via e-mail to Michael.wenrick@us.af.mil with the solicitation number in the subject block. The government reserves the right to evaluate proposals and award a contract without discussions with offerors. Facsimile proposals will not be accepted. Offers shall be evaluated in accordance with FAR 52.212-2, Evaluation of Commercial Items (Jan 1999) which is incorporated into this Request for Proposal below. The Government anticipates the award of three or less contracts from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Technical capability (ii) Price. Technical capability is approximately equal to price. The Government will award a Firm Fixed Price - Blanket Purchase Agreement to the technically-acceptable and responsible offeror(s) with the lowest priced offer. The offeror shall submit a completed "Offer scheduled"(attachment 2) and sample event "Sample Bid Schedule" (attachment 4) with pricing to be evaluated. Actual events will solicited solely among the final BPA recipients and issued as BPA Calls to the original agreement. Technical acceptability will be determined by the following: 1. Lodging must meet the specifications identified in the Statement of Work. Lodging includes the offerors proven capability to supply a minimum number of rooms in the proper room sizes (as identified in the offer schedule for price evaluation), amenities, and patron parking. Lodging rooms shall be in a centralized location where patrons are not subjected to the elements for more than 5 minutes while traveling to meeting space and childcare locations. Additional travel requirements such as shuttle/bus transportation may be offered free of charge and shall be at the will of the patrons and not require pre-arrangement. 2. Meeting space must meet the specifications listed in the Statement of Work to include a minimum of at least two (2 or 3 depending on size of event) conference rooms. 3. Childcare must be provided in accordance with the Statement of Work. Space must be provided for children in a separate location from adults. 4. Meal choices/types must meet the specifications listed in the Statement of Work. Meals must be provided on-site. 5. Pricing must be provided for all specified CLINS. Offer Schedule shall be filled out for both peak and off season rates. Additionally, the contractor shall identify date ranges of peak seasons. The offeror shall also submit pricing for the sample event "Sample Bid Schedule". These prices will be used to evaluate your pricing against other offerors proposals. The "Total Offer Amount" price shall be used as a price evaluation factor for award of two (2) small businesses BPA awards. Pricing shall be proposed using standard off season rates. 6. The lodging facility shall be within 50 miles of the following cities; Charleston, SC; Charlotte, NC; Williamsburg, VA or 75 miles from Myrtle Beach, SC. 7. Offeror CAGE Code shall be the CAGE code for the actual lodging facility and/or lodging facility parent company, LLC, or owner.
