Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Business; e.g., Large Business, Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other small businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name, (b) address, (c) point of contact, (d) phone, fax and email, (e) DUNS number, (f) Cage Code, (g) Tax ID number, (h) type of business, e.g., Small Business, Veteran owned, etc. (i) a description of any GSA Schedule contracts held, and (j) a capability statement that addresses qualifications and ability to perform as a contractor for the work described below. Draft Performance Work Statement (PWS) NAVAHCS Laundry and Uniform Services 1.0 Background: The Northern Arizona VA Health Care System (NAVAHCS) located at 500 North Hwy 89, Prescott, Arizona, 86323, has a continuing requirement for a contractor to provide and maintain work uniforms, laundry and linen services, and other linen items necessary for the operation at NAVAHCS. 2.0 Requirements: The contractor shall provide all management, supervision, personnel, labor, tools, supplies, materials, transportation, equipment, and other items necessary to provide and maintain laundry and uniform services at NAVAHCS. 2.1 NAVAHCS requires uniforms to be supplied in various sizes, quantities, and for various work schedules. (See Appendix A) 2.2 The contractor shall pickup and deliver all soiled uniforms and microfiber mops, and replace with clean, laundered items once a week. This schedule shall be coordinated with the Contracting Officers Representative (COR) during normal duty hours, Monday through Friday, 7:30am to 4:00pm. NOTE: Holidays: When a holiday occurs on a scheduled pick up or delivery day the scheduled pick-up and delivery will be performed on the first work day following the holiday (Monday through Friday). 2.3 The contractor shall count and track all soiled and clean items that leave the facility. It is expected that the number of soiled uniforms collected in one week are the same number of clean uniforms delivered the next week. 2.4 All soiled uniforms picked up from the previous week will be cleaned, pressed and delivered to the appropriate place within one week. 2.5 The contractor shall supply new employees with new uniforms within 14 calendar days of order. 2.6 The contractor shall repair soiled uniforms that need repair or replace garment within one week of pick up. 2.7 The contractor shall replace uniform garments due to fair wear and tear within one week of being returned. Replacements will be new or nearly new in quality. 2.8 The contractor shall provide separate containers for clean and soiled uniforms and microfiber mops. 2.9 Initial uniform issues will consist of 11 sets for each employee working a 5 day work week. All initial uniform issues and subsequent new employee issues will be NEW UNIFORMS. 2.10 The contractor shall provide all uniforms with the following patch and a patch identifying the department the employee works in, for example: Food Services, Environmental Services, SPS, etc.: 3.0 Delivery Locations: NAVAHCS delivery locations for uniforms and microfiber mops are: "Building 31, Maintenance shops "Building 107, basement EMS stock room "Building 107, basement SPS locker room "Building 107, Laboratory "Building 107, Surgery "Building 107, ICU "Building 108, basement F&NS locker room "Building 111, Boiler plant "Building 112, Laundry "Building 117, Emergency room "Building 155, Dental 4.0 Packing: Packaging shall be in accordance with industry standards for all laundry with the exception of unserviceable and re-performance items. 4.1 Unserviceable Articles: All unserviceable articles and non-repairable articles shall be returned in a separate marked bundle, clearly identified as "unserviceable" by the contractor to the Government after laundering. 4.2 Re-performance Items: Any article found to not meet the requirements shall be re-cleaned at no additional cost to the Government. After the articles are re-cleaned, the contractor shall pack these articles separately and identify them by marking "Re-performed." 5.0 Performance Matrix: The contractor shall be evaluated based on the following and all services must adhere to VHA Handbook 1850.04, Employee Uniforms, dated November 4, 2011: ServiceStandardSurveillance Pickup/Delivery ServicesPickup/Delivery accomplished within the hours specified. (0 defects)Customer Complaint UniformsFinished articles conform to accepted industry standards of quality, cleanliness and appearance (free of odors and stains). 0 defects.Customer Complaint Ordering of new UniformsOrders for New Uniforms may be submitted at any time and are expected to be provided for the new employee within 14 calendar days. (1 defect per quarter).Customer Complaint 6.0 QUALITY CONTROL: The contractor shall develop and maintain a quality control program to ensure laundry services are performed in accordance with the terms of this contract and commonly accepted commercial practices. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. ?
