- Revised Channel Listing - Attachment 5 Revised Pricing Schedule - Attachment 2 Revised Statement of Work - Attachment 1 This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is FA4830-14-Q-0004 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-71, Defense Acquisition Circular (DAC) 91-13 (Correction) and Air Force Acquisition Circular (AFAC) 2013-0827. The North American Industry Classification System (NAICS) is 517410. The small business size standard is $30 million. This acquisition will be set aside 100% for small businesses in accordance with Federal Acquisition Regulation (FAR) 19.502-2(a). Description: The purpose of this solicitation is to acquire Satellite Television (SATV) Services for Moody AFB in accordance with the attached Statement of Work (SOW). See Attachment 1- Statement of Work Quotations must be submitted on the attached Pricing Schedule for the following CLINs: See Attachment 2 - Pricing Schedule for Contract Line Items To be determined technically acceptable the quoter must submit a copy of their Satellite Broadcasting and Communications Association (SBCA) certificate with their quote and a list of channels that will be provided for each CLIN in the pricing schedule. The quoter must also be able to demonstrate they are authorized by Direct TV, Dish Network, etc. to be a SATV provider of service for the Moody AFB Area (Zip Code: 31699). Any "teaming" arrangements utilized in the performance of this contract shall be provided to the Contracting Officer in writing at the time of award. The definition of "teaming" is as follows: The process the Contractor uses to select key suppliers and business partners; form mutually beneficial relationships; and, integrate business operations into an organizational structure designed to meet all contract requirements. Note: Wage Determination does not apply. FOB Point: Destination (Moody AFB, GA.) Provision at FAR 52.212-1, Instruction to Quoters Commercial Items applies to this acquisition. Request for Quotation (RFQ) should be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF QUOTES; NAME, ADDRESS; TELEPHONE NUMBER OF QUOTER; TERMS OF THE EXPRESSED WARRANTY; ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Contractors shall insert unit prices in the attached pricing schedule and round to the nearest cent (for example $0.02 not $0.0231) and sign below indicating prices to be used. By signing, the quoter agrees with all terms, conditions, and provisions included in the solicitation. Quotes shall also contain all other documentation specified herein. PERIOD OF ACCEPTANCE OF QUOTE: The quoter agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of quotes, unless another time period is specified in an addendum to the solicitation. LATE QUOTES: Request for Quotations or modification of quotes received at the address specified for the receipt of quotes after the exact time specified for receipt of quotes will not be considered. EQUIPMENT: SATV Equipment (to include Antennas, Receivers, etc...) is currently installed throughout Moody AFB. The government makes no certification on the ability to use the current equipment by the potential awardee. Quoter is responsible for providing Equipment IAW the SOW and/or can use existing equipment. EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order to the quoter whose technically acceptable offer is judged to represent the best value to the Government.
