- FAR 52.212-5 (Dec 2013) Miller-Leaman Pricing Spreadsheet Delivery, Packaging, and Inspection Requirements FAR 52.212-3 (Nov 2013) PLEASE READ THIS NOTICE IN ITS ENTIRETY INCLUDING ATTACHMENTS BEFORE CONTACTING THE POC WITH QUESTIONS/COMMENTS. THANK YOU. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is HSCG40-14-Q-50144 and is issued as a Request For Quotation (RFQ). This announcement incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-72 (dated 1 JAN 2014) and as supplemented with additional information included in this notice. This acquisition is unrestricted and only qualified offerors may submit bids. The NAICS code for this procurement is 333999 with a small business size standard of 500 employees. The U.S. Coast Guard Asset Project Office (APO) has a requirement to procure the items on the attached Miller-Leaman Pricing Spreadsheet (Attachment I). It is anticipated that a non-competitive sole source purchase order shall be awarded as a result of this synopsis/solicitation. It is the Government's belief that only Miller-Leaman, Inc. and/or its authorized resellers possess the necessary technical and engineering data to successfully manufacture this item. All responsible vendors may submit a quotation, which if received timely, shall be considered by the Agency. The attached Delivery, Packaging, and Inspection/Acceptance Requirements (Attachment II) need to be viewed and adhered to. In addition to completing Attachment I, vendors must have a valid DUNS number, be registered with the System for Award Management (SAM) and shall provide the company Tax Information Number (TIN) with its quote. Quotes will be evaluated on price only, and award will be made to the vendor whose quote is the lowest price technically acceptable. Delivery will be FOB DESTINATION to address: U.S. Coast Guard Surface Forces Logistics Center Receiving Room, Bldg #88 2401 Hawkins Pt Rd Baltimore, MD 21226 The following FAR Provisions and Clauses apply to this solicitation. Offerors may obtain full text versions of these provisions and clauses electronically at http://www.arnet.gov/far. FAR 52.212-1, Instructions to Offerors-Commercial Items (July 2013); FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Nov 2013) with Alternate I (included as Attachment III) - vendor must include a completed copy of FAR 52.212-3 with its quote; FAR 52.204-7, System for Award Management (July 2013); FAR 52.212-4, Contract Terms and Conditions--Commercial Items (Sept 2013); FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (Dec 2013) applies to this acquisition, and is included as Attachment IV with the applicable clauses marked. A formal notice of changes, if applicable, will be issued in FedBizOpps (www.fbo.gov) via amendment(s). It is each vendor's individual responsibility to monitor the FBO website for changes.
