- Glenmont Job Corps Center Scope of Work ID # GLS14-001 To Provide Dental Services to Student of Glenmont Job Corps Center. To complete the following Table of Contents Proposal Information Project Description Specific Requirements Proposal Form Proposal Requirements Project: To provide Dentist Service to Glenmont Job Corps Center Students Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator: Adams and Associates, Inc Mailing address Glenmont Job Corps Center 822 River Road Glenmont, NY 12077 Proposal Due Date: 3 PM on 1/29/2014 Payment: Payment Terms are Net 30 days of completion and receipt of Invoice. Project Description Scope of Project: The undersigned Center Dentist agrees to provide students of the Glenmont Job Corps Center with the services stated herein. Such services will be provided as authorized by the Center Director or his/her representative. The Center and the Center Dentist hereby agree that the services to be provided will include, but will not be limited to those oral health and wellness services specified in the Job Corps Federal Regulations (Title 20, Code of Federal Regulations 638.510; the Job Corps Policy and Requirements Handbook, Chapter 6, Section 6.10, R2; and applicable Technical Assistance Guides). The Center Dentist shall oversee the development, implementation, and monitoring of the total Center oral health and wellness program, which is subject to the approval of the Center Director. The hours per week associated with Dental services are as follows: Dentist 8 Hours per week Dental Hygienist 8 Hours per week Dental Assistant 11 Hours per week Center Dentist shall provide adequate licensed personnel to perform dental services for students, including but not limited to: A. Assessment and diagnosis, to include: • Dental readiness inspection within 14 days after entry by the center dentist or designee (i.e., dental hygienist or dental assistant) to identify urgent care need for oral conditions that if not treated are expected to result in dental emergencies in the near future. • Elective oral examination upon student request, including x-rays to precede dental treatment. B. Treatment, to include: • Dental procedures to treat oral disease and correct oral health conditions that my represent employability barriers. Specific procedures include: 1. Restorations 2. Extraction of pathological teeth 3. Root canal therapy on anterior and other strategic teeth 4. Replacement of missing upper anterior teeth with a removable prosthesis 5. Dental hygiene treatment that involves nonsurgical periodontal care to treat periodontal disease. • Referral to off-center facilities as necessary for emergent or urgent condition treatable beyond the expertise of general dentist. C. Oral disease prevention education and management, to include: • Oral strategies, such as oral hygiene instruction, risk assessments, and group education. • Oral health promotion activities with an emphasis on overall wellness and employability. D. Providing a written diagnosis and treatment plan as part of an oral health and wellness plan for each student and obtaining agreement for voluntary care before proceeding with treatment. E. Providing oral health education. F. Establishing an appropriate referral system to ensure the receipt of specialty care as defined in the PRH and within budget constraints. G. Providing or arranging for 24-hour emergency coverage. H. Participating in the coordination and integration of the oral health program with the wellness components, Center activities, and Center-community activities. I. Maintaining the accuracy and confidentiality of all required oral health and wellness records when they are in the subcontractor's presence. J. Providing support to the students in acquiring and practicing the oral health-related skills, knowledge, and attitudes that will make them employable. K. Developing and complying with Job Corps infection control policies and procedures. L. Reviewing and signing Health Care Guidelines annually for dental care in accordance with the PRH requirements. M. Enforcing appropriate student workplace behavior when students are in the dental office or waiting area. N. Arranging appropriate medical separations in conformity with the PRH. O. Collecting data and preparing reports as required by the Center Director and/or the Department of Labor. P. Advising/consulting as requested with the Center Director and Center staff on oral health and dental programmatic issues. Q. Cooperating with corporate and Regional Office Center assessments. R. Ensuring that regular preventive maintenance is provided to the dental equipment. The Center Dentist agrees to adhere to the policies, procedures, and guidelines established by Job Corps. It is understood and agreed that the services provided by the Center Dentist are subject to monitoring and review by the Department of Labor. OBLIGATIONS OF THE PROFESSIONAL In the performance of the above duties and responsibilities, the Center Dentist: A. Agrees to maintain malpractice insurance to cover the professional services to students, with limits consistent with the medical service industry, but in no case less than $1 million per occurrence. B. Agrees to make no public statements regarding its activities under this Subcontract, or to disclose any information regarding Job Corps students or Job Corps activities without prior written approval of the Center Director or his/her designated representative. Unauthorized release of student records is a federal crime punishable by fine and/or imprisonment. C. Shall be responsible for finding and arranging for a professionally qualified replacement/substitute during temporary periods of absence of more than one calendar week. D. Shall be personally and financially liable for the cost of any patient referral outside the Center without prior written approval of the Center Director. E. Certifies its intent to comply with all equal employment opportunity practices and affirmative action policies as required by law and agrees not to discriminate against any student, employee and/or contractor because of race, religion, color, gender, age or national origin. F. Shall provide current copies of all professional licenses, certifications and professional liability insurance documents required by the Center. G. Agrees that all research projects and/or publications relating to Job Corps health activities will first be cleared through the National Office of Job Corps. Proposal Form The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID #GLS14-001 and address all items, to be considered. The Contractor shall submit the following information with the proposal: References Copies of Insurance Certificates and applicable licenses. Completed vendor questionnaire and W9.
