- NPTU Security Form JB CHS- Contract Security Form 74 - SAMPLE JB CHS- Contract Security Form 74 Map to Redbank Club UPDATE AS OF JANUARY 9, 2013************************************* The RFP is anticipated to be released within the next few days. In preparation for the pre-proposal conference and site visit scheduled for January 28th, information is provided below. Once the RFP is posted, refer to FAR 52.236-27 Site Visit (Construction) for any changes or updates to site visit information. In addition comments to the draft solicitation have been posted under the special notice on posted to Federal Business Opportunities (FedBizOpps) at https://www.fbo.gov under the title: DRAFT Solicitation and Industry Forum for P-100/P-099, Nuclear Power Training Facilities, Joint Base Charleston, South Carolina. SITE VISIT INFORMATION An organized site visit has been scheduled for-- January 28, 2014 at 0900 Participants will meet at-- The Redbank Club 2316 Redbank Rd, Bldg 942, Goose Creek, South Carolina 29445 Additional Information Firms participating in the Pre-Proposal Conference and site visit will not be directly compensated for their participation. The Conference will be held Tuesday, January 28, 2014 from 9:00 am to approximately 12:00 pm at the Redbank Club located before the Redbank Road entrance to Joint Base, Charleston, SC. A badge is not required for participants only attending the pre-proposal conference. A map illustrating the location of the Redbank Club is provided. Please note that the speed limit and cell phone use along Redbank Road is STRICTLY enforced by Base Security. Cell phone use is strictly PROHIBITED unless using a hands-free device. The site visit will be conducted immediately following the Pre-proposal Conference. This will be the only site visit. Individual requests to re-visit the site at a later date will not be considered. Those attending the site visit will be bussed from the Redbank Club to the construction site and will be escorted around the construction site up to the NPTU controlled perimeter. In addition, one person from each general contractor or waterfront contractor will be allowed to access into the NPTU controlled perimeter. There are no pictures allowed inside or outside the fence. A base access form (JB CHS- Contract Security Form 74) will be required for all attendees who will attend the site visit and a hardcopy must be received no later than January 17, 2014. Refer to Base Access instructions below for further details. Ensure all instructions are followed. Failure to provide a complete and accurate form may result in denied access to the base. Therefore, it is highly advised that forms be submitted as early as possible to prevent any issues. A second access form (NPTU Security Form) will be required to enter the NPTU controlled site. Again, only one person from each general contractor or waterfront contractor will be allowed to access the NPTU controlled site. This form must be received no later than January 17, 2014. Refer to NPTU Site Access instructions below for further details. This is an industrial site, so dress appropriately, i.e. no open toed shoes, etc. Attendees are not allowed to bring cellphones, cameras, flash drives, cds, etc into the site. There are no pictures allowed inside or outside the fence. Only United States citizens will be allowed access into the NPTU controlled site. Base Access Instructions: (A) Offerors must complete the JB CHS- Contract Security Form 74 (attached) for each person requesting access to the base. A sample form is provided as an example (JB CHS- Contract Security Form 74 Sample). (B) Top portion needs to be complete in addition to #1 through #23. SSAN refers to the person's Social Security Number. The following have been pre-filled: #5, #6, #9 through #15, #18, and #19. (C) Contractors must print/provide the document in COLOR (not black/white or grayscale) along with a legible copy of drivers license. (D) Original Wet Signatures (no scanned or copied) signatures are required so forms must be mailed via UPS/FedEx/etc. or delivered to our office: ROICC Charleston Attn: Mike Morgan (Badge Request) 2120 Quality Circle, Building 71 Goose Creek, SC 29445 (E) Original wet initials must be included on page 2 of the Form 74 under item 20. (F) Forms need to be delivered to ROICC Charleston no later than January 17, 2014. We cannot guarantee any forms received after this timeframe will be processed. (G) A badge is not required for participants only attending the pre-proposal conference. NPTU Site Access Instructions (A) Offerors must complete the NPTU Security Form (attached) for each person requesting access to the current NPTU site. Again, only one person from each general contractor or waterfront contractor will be allowed to access the NPTU site (B) Forms shall be sent electronically to Kendra McMahon at Kendra.mcmahon@navy.mil no later than January 17, 2014. We cannot guarantee any forms received after this timeframe will be processed. (C) Only United States citizens will be allowed on site.
