Description

This is a sources sought notice to determine the availability of potential sources having the skills and capabilities necessary to provide Home Oxygen Services for the veteran beneficiaries of the VA Medical Center in Erie, Pennsylvania. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 532291, Size Standard $30.0 Million. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will need to respond to the future solicitation announcement. For purpose of this Sources Sought Request for Information, provide the information requested below. The VA Medical Center - Erie, Pennsylvania is seeking sources for a potential contractor to provide Home Oxygen Services to its veteran beneficiaries. Interested sources are required to submit an e-mail delineating qualifications, capabilities, and experience for providing Home Oxygen Services. Responses to this notice should include the following: name, address, telephone number, email address of company/provider; DUNS Number, business size; socio-economic category, if any; firm's experience in providing home oxygen services which demonstrates firm's ability to meet the requirement. Include past experience in performing these services to the VA, other Government (Federal or State) agency, or for a private facility. Include contract numbers, dollar values, points of contact, email addresses, telephone numbers of individuals that can verify this information Please specify your availability to start and timeframe required to initiate services. Responses must be emailed to Cynthia.grygier@va.gov no later than 2:00PM EST, Friday, January 17, 2014. Your response should include both the STATEMENT OF CAPABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained above. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Draft Scope of Work The contractor shall provide Home Oxygen Services for the Erie VA Medical Center (Erie VAMC), 135 East 38th Street, Erie, Pennsylvania. This contract will be for a period of one (1) year from date of award. The acquisition of Home Oxygen Services is being initiated to provide these services to veteran beneficiaries serviced by the Erie VAMC. Contractor shall provide Home Oxygen Services to any patients that are treated at the Erie VAMC and all of its Community Based Outpatient Clinics (CBOCs) in strict accordance with the performance work statement. Areas of service for the Erie VAMC include, but are not limited to the following counties: Counties in PA: Erie, McKean, Crawford, Venango, Mercer, Warren, and Forrest Counties in OH: Ashtabula Counties in NY: Chautauqua, Cattaraugus The need includes providing in-home oxygen supplies such as the following: CPAP/BIPAP machines and replacement parts; rental of Concentrators and replacement parts (such as regulators, carriers, shoulder bags, humidifiers), rental of various cylinders (including sizes A, D, E, and M) and replacement parts (such as nasal cannula/masks); and portable oxygen systems. The services associated with this contract include: All logistical/administrative functions involved with the day to day operation of providing Home Oxygen Services to the Veteran and their families, including in home respiratory therapy visits. This includes providing a Quality Control Plan (QC) plan, coordinating and managing patient care, deliveries consisting of pickup and delivery of supplies, briefing patient on proper use of equipment, documenting use of supplies and inspection of home in accordance with Joint Commission standards concerning patient use/safety of home oxygen.
