VA69D-14-I-0383; Source Sought Notice for Iron Mountain Ambulance Service Jessica L. Howard, Contract Specialist, NCO 12 GLAC 1/16/2014 The Oscar G. Johnson VA Medical Center, 325 East H Street, Iron Mountain, MI 49801 is seeking potential sources capable of providing sufficient labor, equipment, and materials that include drivers, ambulances, paramedics, and attendants, when required, to provide Basic Life Support (BLS) and Advanced Life Support (ALS) ambulance service for the beneficiaries of the Department of Veterans Affairs Oscar G. Johnson Medical Center (VAMC), Iron Mountain, Michigan. Contract services includes, may include but are not limited to, trips between VAMC Iron Mountain, Community Based Outpatient Clinics, patient residences, private hospitals and Department of Veterans Affairs Medical Centers within VISN 12 Emergency Medical Technicians (EMT) providing emergency service on ambulances under this contract must have completed training in accordance with the standards published by the Department of Health and Human Services. EMTs shall be certified, licensed or otherwise officially recognized by the local, state or regional government or public entity where the emergency ambulance service is operated or by which it is governed. Ambulance drivers shall have a valid operator's or chauffeur's license in accordance with Federal, State and local government requirements for their place of operation, for the services they perform and be capable of administering oxygen. The applicable NAICS Code is 621910 with a size standard of $14.0Million. No formal solicitation document exists at this time. Responses to this sources sought notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned (certified on VIP); firm's experience in providing emergent and non-emergent, basic and advanced life support ambulance service in a professional setting and any other pertinent information, which demonstrates ability to meet the above requirement.
