- Wage Determination 2005-3007, Rev. 14 Statement of Work RFQ Template Memorandum for Interested Parties This is a combined synopsis/solicitation for commercial services. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation reference number F4ATA73339A001 is issued as a Request for Quotation (RFQ). This acquisition is 100% set aside for small businesses under North American Industry Classification System (NAICS) code 485510. The small business size standard is $14.0 Million. It is the intent of the Government to set up multiple Blanket Purchase Agreements (BPAs) for charter bus services from Tyndall AFB, FL to the following destinations: Norfolk, VA; Baltimore, MD; or Charleston, SC. EVALUATION FACTORS: In accordance with FAR 52.212-2, Evaluation-Commercial Items, the Government intends to set up multiple BPAs resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factor(s) considered. The following factors shall be used to evaluate offers: 1. Technical Capability 2. Price TERMS AND CONDITIONS BLANKET PURCHASE AGREEMENT IAW FAR 13.303 1. DESCRIPTION OF BLANKET PURCHASE AGREEMENT (BPA): This is a pre-priced agreement issued to allow Tyndall Air Force Base an acquisition tool to purchase charter bus services for 823 RHS operations at Tyndall AFB, FL. This agreement is effective upon signature of the Contracting Officer and will expire 30 January 2019. This agreement may be terminated unilaterally at any time by either party upon 30 days written notice and may be rewritten or revised at the option of the Contracting Officer. Any such revision shall be accomplished by modification to this BPA. Obligation is only incurred against this order when a call is placed for a specific item and amount. Payment is made after delivery of each call placed during that month. Calls are placed and only authorized by personnel identified in writing by the Contracting Officer. 2. EXTENT OF OBLIGATION: The government is obligated under this BPA only to the extent of the authorized calls actually placed by personnel identified by the Contracting Officer. 3. PRICING: Prices of this BPA are set by the contractor's published price list as approved by the Contracting Officer. BPA prices shall be no greater than those prices offered to preferred customers and shall not exceed the maximum BPA call limit. Contractor shall notify the Contracting Officer of any changes pertaining to the terms and conditions of this BPA. 4. PURCHASE LIMITS: Calls against this BPA shall not exceed $15,000.00 unless specifically approved by the Contracting Officer. 5. INDIVIDUALS AUTHORIZED TO PLACE CALLS UNDER THIS BPA: A list of personnel authorized to place calls will be provided to the Contractor, in writing, after award by the Contracting Officer. 6. DELIVERY TICKETS: All deliveries and/or shipments under this BPA shall be accompanied by delivery tickets or sales slips that shall contain the following minimum information: (i) Name of supplier. (ii) BPA number. (iii) Date of purchase/call. (iv) Purchase number/call number. (v) Itemized list of services furnished. (vi) Quantity, unit price, and extension of each item, less applicable discounts (unit prices and extensions need not be shown when incompatible with the use of automated systems, provided that the invoice is itemized to show this information). (vii) Date of delivery or shipment. 7. PAYMENT: Payments shall be made via the Wide Area Work Flow (WAWF). 8. INVOICES: Invoices shall be attached in WAWF. In accordance with FAR 13.303-3(a)(6)(ii), "An itemized invoice shall be submitted at least monthly or upon expiration of this BPA, whichever occurs first, for all deliveries made during a billing period and for which payment has not been received. These invoices need not be supported by copies of delivery tickets.' 9. DELIVERY DATE FOR CALLS: Persons authorized to place calls will establish a delivery date with the contractor for each call placed against this BPA. 10. WAGE DETERMINATION: See attached Wage Determination: 2005-3007 Rev. 14. 11. PERIOD OF PERFORMANCE: Will be approximately 1 February 2014 - 30 January 2019.
