- Intent to sole/limited source The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 3), Network Contracting Office 3 (NCO 3), intends to award a Limited Sources contract under the authority of FAR 8.405-6 Limited Sources Justification to Philips Healthcare, a Division of PENAC, located at Bothell-Everett Highway, Bothell, WA 98041, due to limited sources available for the Philips Healthcare medical equipment imaging maintenance services. Philips Healthcare, a Division of PENAC, will furnish all service and maintenance for the medical equipment imaging maintenance services. This action will result in a Firm-Fixed Price contract with a period of performance of twelve (12) months. This notice of intent is not a request for quotations; a determination not to compete this proposed contract upon responses to this notice is solely within the discretion of the Government. Verbal responses are not acceptable and will not be considered. This notice of intent to award a sole source contract is not a request for competitive quotes. There will be no solicitation available for competitive quotes. Phone calls will not be accepted. The point of contact for this action is Contract Specialist, Lawrence Unger and can be reached via lawrence.unger@va.gov TASKS TO BE COMPLETED Contractor will furnish full medical equipment imaging maintenance services to include repair to any task directly related to issues with multiple Philips Imaging and therapy systems including; D76/15"/S80CP/DSI SYSTEM,SCOPOMATIC 76 SYSTEM, VERTEX SYSTEM,GENSYS SYSTEM, X-RAY SYSTEM, ULTRASOUND SYSTEMS WITH TRANSDUCERS M#C5-2, OMNI TEE PROBES, XCELERA SYSTEM WITH ECHO VIEWING/REPORTING AND SERVER SOFTWARE, AS WELL AS REQUIRED COMPONENTS TO MAINTAIN THE SYSTEMS LOCATED AT THE NYH-MANHATTAN VETERANS AFFAIRS MEDICAL CENTER. BRILLIANCE BIG BORE, PHILIPS MEDICAL SYSTEMS CUSTOMER SERVICE AGREEMENT FOR 2 INDEPENDENT WORKSTATIONS, C-ARM BV PULSERA AND 300 SYSTEMS, VERADIUS C-ARM UNIT, ULTRASOUND SYSTEMS, PINACCLE SERVER, AS WELL AS REQUIRED COMPONENTS TO MAINTAIN THE SYSTEMS LOCATED AT THE NYH-BROOKLYN CAMPUS VETERANS AFFAIRS MEDICAL CENTER. The contractor will deliver all questions and question sets, license documents if any, and confidentiality, use and non-use agreements as set forth in the agreed Schedule of Performance. VHA will deliver all confidentiality, use, and non-use agreements as set forth in the agreed Schedule of Performance. A written contracted order for the specified tasks at each site will be distributed to the Sole contracted source only. PERIOD OF PERFORMANCE The period of performance for this contract is one base year, effective February 2014.
