- The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 3), James J. Peters VA Medical Center, Bronx New York, intends to award a sole source contract under the authority of 41 U.S.C 253(c) (1) - FAR 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements, to Philips Helathcare, 22100 Bothell Everett HWY, Bothell, WA 98021. Philips Healthcare will lease a 101869 iU22 Preowned System. To include NNAJ105 DS iU22 2012, NUSF381 Radiology Clinical Package, NUSF330 ER L9-3 Transducer, NUSF331 ER L12-5 Transducer, NUSF340 ER C5-1 Transducer, FUS7000 English Manual. The place of performance is Hudson Valley Healthcare System, Castle Point Campus, Rt. 9D, Castle Point NY, 12511. This action will result in a Firm-Fixed Price contract with a period of performance of twelve (12) months with four (4) option years. This notice of intent is not a request for quotations; Interested parties may express their interest by providing a capabilities statement NLT 23 January at 8:00 AM EST. The capabilities statement must provide clear and unambiguous evidence to substantiate the capability of the party to provide the required supplies and services. A determination not to compete this proposed contract upon responses to this notice is solely within the discretion of the Government. Verbal responses are not acceptable and will not be considered. When responding to this announcement, respondents should refer to Announcement VA243-14-Q- 0375 Notice of Intent. If after January 23, 2014, no viable responses have been received in response to this announcement, Dept of VA shall negotiate solely with Philips Healthcare. This notice of intent to award a sole source contract is not a request for competitive quotes. There will be no solicitation available for competitive quotes. Phone calls will not be accepted. The point of contact for this action is Contract Specialist, Tina Alves who can be reached at Tina.Alves@va.gov. The Government anticipates the award of a firm-fixed- price contract. A determination by the Government whether to compete this proposed requirement based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competition. A.GENERAL INFORMATION 1. Title of Project: Leased Ultrasound System 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW). The purpose of the Leased Ultrasound System is to facilitate a reduction in scheduling backlog thus reducing the possibility of delayed diagnosis. The contract must reference that the hard drive remains at VA Hudson Valley and will never be returned to the vendor for any reason. 3. Background: Ultrasound Systems have been in use for almost forty years in hospitals, imaging centers and VA facilities. The VA Hudson Valley currently owns two Ultrasound Systems and has experienced significant exam volume growth over the past 10 years. Currently the VA Hudson Valley has two Ultrasound Systems and cannot keep pace with the increasing demand which has left us with a four month appointment backlog. Adding a third system now would enable us gain ground and overtime eliminate the appointment backlog. 4. Performance Period: The period of performance is to deliver the product within 30 days of award. 5. Type of Contract: Lease 6. Place of Performance: VA Hudson Valley, Castle Point Campus, Radiology Department. 7. Performance Based Contract: This contract requires the application of an Earned Value Management System that is compliant with the American National Standards Institute /Electronics Industries Alliance (ANSI/EIA) Standard-748, Earned Value Management Systems. [See paragraph Q.] B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, together with associated submilestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed project management plan (PMP). The following Ultrasound system and associated components are required; a.Refurbished Ultrasound System capable for performing General and Vascular Ultrasound Exams. b.The following transducers; L9-3, L12-5, C5-1 c. DICOM print, store, link to network d. Operators Manual 2. All written deliverables will be phrased in layperson language. Statistical and other technical terminology will not be used without providing a glossary of terms. 3. Where a written milestone deliverable is required in draft form, the HVHCS will complete their review of the draft deliverable within 7 calendar days from date of receipt. The contractor shall have 7 calendar days to deliver the final deliverable from date of receipt of the government's comments.
