- PWS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FA4861-14-T-0002 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-71, Defense Publication Notice 20131206, and Air Force Acquisition Circular 2013-0827. (iv) The solicitation is 100% set-aside for small business concerns. The associated NAICS code is 515210, All Other Telecommunications and the small business size standard is $30.0 million dollars. (v) There is only one contract line item number (CLIN) per performance period. The base year CLIN is 0001 with a performance period of 14 February 2014 through 13 February 2015 for a quantity of 12 months. There is four additional option CLINS, 1001,2001, 3001, 4001 with a quantity of 12 months for each option period. A price must be provided for all CLINs or the quotation may not be considered for award. (vi) The description of the requirement is: The contractor shall construct, install, operate and maintain cable and phone services at the proposed TLF locations on Dunning Circle and Kinley Drive, Nellis AFB, Nevada in accordance with the terms, conditions, and standards set forth in this performance work statement (PWS). The Contractor shall be capable of providing service to 6441, 6451, 6461, 6471, 6481 and 6501 Dunning Circle. The Contractor shall be capable of providing service to the units located at 4828, 4830, 4832, 4834, 4836, 1838, 1842, 4844, 4846, 4848, 4851, 4852, and 4854 Kinley Drive. All materials required for installation, operation, and maintenance of the cable and phone services to the building must be furnished by the Contractor at its own expense. (vii) Service to the TLF proving cables and phones shall begin 14 February 2014. FOB point is destination. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) The provision at 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. The Lowest Price Technically Acceptable Source Selection Process is used in this acquisition. The specific evaluation criteria are price and technical acceptability as it relates to the Performance Work Statement. (x) The provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, must be provided with the quotation. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision. (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. An addendum is made to paragraph (o). The contractor is required to provide the government with all standard commercial warranties. (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. Since this is a RFQ, specific clauses that apply to this acquisition will be included in the Government Purchase Order. (xiii) The government recently had service through OLCR INC. of Chester, Pennsylvania. OLCR Inc provided content service through a subcontract with DirecTV. The subcontractor that provided service through the Phone Lines was Century Link. DirecTV and/or Century Link is not a mandatory source for this acquisition. (xiv) The Defense Priorities and Allocations System (DPAS) do not have a rating assigned to this acquisition. (xv) Quotations are due by 1200HRS PST, 05 February 2014.
