- The Department of Veterans Affairs, SAO Central, NCO12 - Milwaukee, WI, intends to award a sole source commercial purchase order in accordance with FAR Parts 12 & 13. The North America Industry Classification System Code [NAICS] is 811219 and standard is $19 Million. The government has determined that Ethicon, Inc DBA Advanced Sterilization Products Division, is the only source that can satisfy this agency requirement. The requirement is provide preventative maintenance on the specified Sterrad equipment, as well as labor, travel, expenses, and parts for that preventative maintenance. Additionally, it provides on-call corrective maintenance on equipment that is malfunctioning or inoperable, as well as labor, travel, expenses, and parts deemed necessary for that corrective maintenance for NCO 12: Jesse Brown VAMC, Federal Health Care Center in North Chicago, IL, Edward Hines VAMC in Chicago, IL, William S Middleton VAMC in Madison, WI, Green Bay Clinic (Milo C. Huempfner), Oscar G Johnson VAMC in Iron Mountain, MI, and Zablocki VAMC in Milwaukee, WI. The contractor must meet The Joint Commission requirements for proving competency of service provider. Contractor personnel shall be OEM factory trained and authorized to work on equipment they are servicing. This synopsis is for notification purposes only and does not constitute a solicitation for bids or proposals. Under Statutory Authority 41 U.S.C. 253(c)(1), the VA Great Lakes Acquisition Center anticipates entering into sole source procurement negotiations with Datex-Ohmeda. This announcement is for services for which the Government intends to solicit and negotiate with only one source under FAR 6.302-1. The above authority is cited and justification made based on the determination that the Government's minimum needs can only be satisfied for supplies/services available from one source. This notice of intent is not a request for competitive proposals. Other persons may identify their interest and capability to respond to this requirement by January 27, 2014 by 2:00 PM CST. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the requirement. All interest received on or before January 27, 2014 will be evaluated by the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government. Information received as a result of the notice of intent will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. No solicitations documents are available and telephone requests will not be honored. Any company that feels they can meet the needs of the Government must submit their technical data (10 pages or less) to support their claim. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. 3) Submissions must be made via email to: trong.nguyen@va.gov The Government does not intend to pay for any information provided under this synopsis.
