- AMENDMENT 0002 CHANGE TO QUOTATION DEADLINE TIME: QUOTES ARE NOW DUE BY 12 NOON, WEDNESDAY, JANUARY 22, 2014. Due to storm activity and the need to guarantee a delivery by Thursday, January 23, 2014, the deadline time is hereby changed to noon today to ensure the awardee has time to schedule a delivery for tomorrow. AMENDMENT 0001 - 1. DEADLINE EXTENDED - QUOTES ARE NOW DUE ON WEDNESDAY, JANUARY 22, 2014 at 5:00 PM, EST. 2. The small business size standard for NAICS 454310 Fuel Dealers was incorrectly stated as 500 employees. Please be advised that the correct size standard for NAICS 454310 Fuel Dealers is 50 employees. Request for Proposal: GS-02P-14-PB-P-0023 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number GS-02P-14-PB-P-0023 　is being issued as a Request for Proposal in accordance with FAR Parts 12 & 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72 effective January 1, 2014. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulation (FAR) accessed on the internet at https://www.acquisition.gov/far/. Total Small Business Set Aside This solicitation is a 100% percent small business set aside. The associated NAICS is 454310 Fuel Dealers and the applicable SBA Small Business Size Standard is 50 employees. Proposals will only be considered from vendors who are a Small Business in accordance with NAICS 454310 and the applicable size standard of no more than 50 employees. Requirement The requirement is for 15,000 gallons of #2 Fuel Oil to be delivered to the FDA Laboratory Facility, located at 158-15 Liberty Ave., Jamaica, NY 11433-1034. Vendors shall submit a quote that expresses their proposed price as a price per gallon unit price, inclusive of all shipping costs and applicable fees. Fuel Oil must comply with all NYC regulations. Quotes must be submitted on company letter head signed by an authorized representative. Please include the firm's DUNS number on the quote. Delivery Date will be Thursday, January 23, 2014. A firm delivery date will be established at the time of award. Evaluation Criteria The potential award will be made to the vendor that offers the lowest overall price. Proposed prices must be valid for 7 days after the due date of the proposals. Notice to Vendors This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Proposals from vendors not registered in Sam will not be considered. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.204-10, 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.222-37, 52.223-18, 52.225-1, 52.225-13, 52.232-33.
