Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number VA528-14-Q-0104 is issued as a request for quotation. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. The applicable NAICS code is 339112 and the size standard is 500 Employees. This RFQ is being issued as Unrestricted. The Contractor shall deliver FOB Destination to the Department of Veterans Affairs, VAMC, Albany, NY, 113 Holland Avenue, Albany, NY 12208-3410. All responsible Vendors shall provide an offer for the following "Brand Name or Equal" Products: Line Item No. 0001: Generator, Gyrus PK-SP, P/N: 744000 or Equal, 1 Each Line Item No. 0002: Footswitch, Gyrus PK-SP, P/N: 744010 or Equal, 1 Each Line Item No. 0003: Cord Angle Connector, Hospital Grade, P/N: 560085-002 or Equal, 1 Each The salient characteristic of the equipment that must be included if quoting Brand Name or Equal is that it MUST be compatible with the Da Vinci Surgical System with HD. The VA Medical Center will make one award to the offeror who provides the best value to the VA based on price and other price related factors contained in this notice; capability of the item meeting the attached specifications' and the offeror's past performance in providing this type of equipment. Offerors are required to submit descriptive literature with their quote. Quotes may be emailed to Teresa.helm2@va.gov. All responsible offerors may submit a quote which shall be considered by the VA Medical Center.
