- STATEMENT OF WORK A.GENERAL INFORMATION 1. Title of Project: Introductory X-ray training (Principles of Servicing Diagnostic X-Ray Systems) 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Contractor shall provide training and certification on Principles of Servicing Diagnostic X-Ray Systems. The vendor shall provide on-site training at the vendor's facility to two (2) Lebanon VAMC X-ray technicians. 3. Background: Currently our biomedical department has to rely on outside vendors to provide service and expertise. This training would allow the medical center to have in-house staff that can diagnose and handle any issues. 4. Performance Period: The contractor shall complete the work required under this SOW during a program of instruction lasting between 80 and 100 hours. If the contractor proposes a program of instruction of less than 80 hours, and the Government accepts the contractor's proposal, the contractor's proposed program of instruction shall prevail. 5. Type of Contract: [Firm-Fixed-Price]. The vendor shall propose a firm fixed price tuition for the two (2) students for the introductory training. B. CONTRACT AWARD MEETING The vendor will acknowledge receipt of the purchase order. The vendor shall have the purchase order signed by the appropriate person authorized and send the purchase order to the contract specialist. The contract specialist will have the purchase order countersigned by the contracting officer. The contract specialist will return the executed purchase order to the vendor. At this point there will be a binding agreement between the vendor and the U.S. Government. C. GENERAL REQUIREMENTS 1. The vendor shall provide a classroom and hands on learning environment which is conducive to learning for the student. The vendor shall provide all learning materials, handouts, and tools required to provide introductory training to the Lebanon VAMC X-ray technicians. The vendor's program of instruction shall be sufficient to provide newly assigned x-ray technicians the knowledge, skills and abilities to understand X-ray systems and their application in the medical community. 2. The program of instruction will have at least six major learning objectives: 1 Introduction to radiography 2 Radiation safety 3 The production of X-rays 4 Formation of the X-ray image 5 Film and film processing 6 Introduction to imaging 3. The course will be comprised of lectures, demonstrations and hands-on training which teach operation, calibration and preventative maintenance of an X-ray system. D. SPECIFIC REQUIREMENTS: 1. The vendor shall provide instructors which are certified for at least the level of instruction that is being provided to the students. The vendor shall submit proof of their certifications. 2. The vendor shall provide classroom and hands on training, lecture materials and handouts and any necessary equipment and tools for the students during the course. 3. The vendor is not responsible for transportation, lodging, and feeding the students. This is the responsibility of the government. A. GENERAL INFORMATION 1. The program of instruction will have at least five major learning objectives: 1 Determine if the X-ray generator system meets the manufacturer's specifications 2 Use proper test procedures to ensure optimum performance 3 Isolate malfunctions to circuit level 4 Perform complete preventive maintenance and system performance checks 5 Perform complete CDRH 3. The course will be comprised of lectures, demonstrations and hands-on training which teach operation, calibration and preventative maintenance of an X-ray system. D. SPECIFIC REQUIREMENTS: 1.
