- AMENDMENT 03 Bulk Oxygen Site Visit Questions A Bulk Oxygen Storage Yard site visit at the new hospital was conducted on 30 Jan 2014 at approximately 9:20 am. The following questions were asked during the site visit and solicitation, please see the answers below. Q: What are the pad dimensions? A: The pad is 21 feet x 32 feet, and is 12 inches thick. The reinforcing is: #4 bars at 12 inches on center in the top of the pad and #7 bars at 8 inches on center in the bottom of the pad. Q: Is there Telemetry involved? A: No telemetry will not be used to monitor the bulk oxygen storage level. A Government Ordering Officer will be appointed and will place orders on a quote mark as needed basis quote mark. Q: Is this a small business set aside? A: No, this is an unrestricted procurement. Q: Is there a SF1449? A: No, the FedBizOps posting was announced as a combined synopsis/solicitation therefore there will be no SF1449. Q: Is there a price evaluation adjustment for qualified HUB Zone Concerns? A: No, an adjustment will not be utilized. OTHER INFORMATION Initial access to install the storage tanks will be via the construction site entrance. Details for entering the construction site and authorization for workers will be provided to the Awardee at time of the award. On site points of contact will also be provided at time of award to the awardee. Upon completion of construction oxygen delivery trucks will utilize the truck entrance to Fort Hood. Per the solicitation, the date to install the storage tanks including certification and filling the 3,000 gallon and the 500 gallon storage tanks is 1 Apr 2014. If there are any construction delays by the Government the Period of Performance start day will be adjusted. AMENDMENT 02 Announces a SITE VISIT on 30 Jan 2014 at 9:00 am CST, to see the Bulk Oxygen Tank yard located at the new hospital construction site on Fort Hood. Visitors will meet at the railhead for the site visit (see directions below). Visitors should wear sturdy closed toe shoes, long pants, and a shirt with sleeves. Note: Allow for adequate time to secure a vehicle pass for access onto Fort Hood. Directions: Exit 190 onto the TJ Mills, stay in the right hand lane Stop at the Visitor Center and secure a visitors pass if necessary. Exit the Visitor Center using the Santa Fe Ramp Stay on Santa Fe stop at the Security Check Point Continue straight on Santa Fe through the first traffic light (the old hospital will be on the right, the new hospital on the left) Turn left at the second traffic light to remain on Santa Fe Turn left on 72nd (by the Recycling Center sign) Continue on 72nd, around the curve, the end point will be at a dead end, parking lot, near railhead Questions pertaining to ONLY the site visit or directions can be directed to 254-432-1000. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued in accordance with FAR Part 12 and evaluated in accordance with FAR Part 13. The Southern Regional Contracting Office (SRCO), Fort Hood, Texas, has a requirement for the installation of a three thousand (3,000) gallon medical grade liquid oxygen bulk storage main tank and a (500) gallon reserve tank with associated equipment (i.e. vaporizer and control equipment), and delivery of medical grade liquid oxygen on a as needed basis for the new hospital at Carl R. Darnall Army Medical Center, Fort Hood, TX. The installation must be completed no later than 1 Apr 2014 and includes: shipping, crane, labor, and certification. The tanks and equipment will be rented and the contractor retains title to the equipment after the rental period. See Statement of Work for details. This announcement constitutes the only solicitation; proposals are being requested. The solicitation number for these requirements is W81K00-14-T-0036. The solicitation is issued as a request for proposals (RFP). This acquisition is subject to the availability of FY14 appropriations. The requirements consist of the following: A list of contract line items number(s) (CLINS) and items, quantities and units of measure, including options CLINS is available on the attachment entitled quote mark Pricing Schedule quote mark. A copy of the Statement of Work is available as an attachment. The Base Year beginning 01 Apr 2014 to 30 Sep 2014 for Filling Bulk Medical Gas and installation of a three thousand (3,000) gallon and a (500) gallon medical grade liquid oxygen bulk storage reserve tank with associated equipment (i.e. vaporizer and control equipment), for the new hospital located at Carl R. Darnall Army Medical Center, Fort Hood, Texas. The contractor shall deliver Medical Grade Oxygen to Carl R. Darnall Army Medical Center, Bldg# 36065, Fort Hood, Texas 76544 on a as needed basis for a base and four (4) - 12 month option periods, IAW the attached Statement of Work, and Pricing Schedule. Please see attachments. SRCO anticipates a FFP contract and intends to make one award for the aggregate of the requirement resulting from this solicitation. The resulting contract will be for one (1) base year with a Period of Performance beginning 01 Apr 2014 through 30 Sep 2014, plus four (4) one-year option periods (01 Apr 2014 through 30 Sep 2018). IAW FAR Part 17, option year pricing associated with FAR Clause 52.217-8 and 52.217-9 will be evaluated at the time of award. Except when it is determined in accordance with FAR 17.206(b) not to be in the Government's best interest, the Government will evaluate offers for award purposes by adding the total price for all options to the total price of the basic requirement for evaluation purposes. The evaluation of option pricing shall not obligate the Government to the exercise of the options. This combined synopsis/solicitation incorporates one or more clauses and provisions by reference with the same force and effect as if they were stated in full text. Full text provisions and clauses can be viewed at http://farsite.hill.af.mil. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. The following instructions are provided to industry: The Government will award a single contract, resulting from this solicitation, to the responsible offeror whose offer, conforming to the solicitation, is the Lowest Price, Technically Acceptable (LPTA). The following factors shall be used to evaluate offers: Technical Capability, Past Performance, and Price. Salient Characteristics: Summary - Install and rent bulk liquid oxygen storage tanks and associated equipment for a base year and four option years. Installation must be completed on or before 1 Apr 2014. The 3,000 gallon medical grade liquid oxygen bulk storage main tank and the 500 gallon reserve tank must fit on the pad - within the designated fenced in area (21 quote mark x 32 quote mark ft), see the attached diagram of the bulk oxygen yard. - Must be Medical Grade Liquid Bulk Oxygen. Must be able to fulfill delivery schedule. The quantities shown in the solicitation schedule are estimates of the facility's annual requirements for the requirement to support a 151 bed medical facility. There is no express or implied guarantee that these quantities will be purchased. - Medical Grade Liquid Oxygen, Contractor shall deliver Medical Gases to Carl R. Darnall Army Medical Center, Bldg# 36065, Fort Hood, Texas 76544 on a quote mark as needed basis quote mark ordered by Government Ordering Officer. Quality Assurance Specifications and Requirements All medical gas manufacturers and fillers of medical gases must be registered with FDA as drug manufacturers. All oxygen shall be manufactured, processed, packed, transported, and stored according to FDA's Current Good Manufacturing Practices (CGMP) regulations, and all labeling shall comply with FDA's labeling regulations (21 CFR Part 201). All liquid bulk oxygen delivered under the contract shall be medical-grade and shall meet or exceed the standards cited in the current edition of the United States Pharmacopoeia/National Formulary (U.S.P.). A valid certificate of analysis shall be provided with each delivery of liquid oxygen. The certificate shall include, at a minimum: a)Supplier's name and complete address b)Name of the Product (i.e. Oxygen U.S.P.) c)An Air Liquefaction Statement where appropriate d)Lot number or other unique identification number e)Actual analytical results for full U.S.P. monograph testing. (A statement that only states that the product meets the minimum purity of 99.5%, etc. is not acceptable.) f) Test method used to perform the analysis. (A statement such as quote mark Meets U.S.P. specifications quote mark is not acceptable; nor would quote mark Tested via Servomex quote mark be acceptable since the specific model number is not provided.) g) Signature of authorized supplier representative and date. Material Safety Data Sheets shall be provided to the facility representative upon request. A copy of all inspection reports shall be provided to the facility representative upon the completion of contractor owned bulk oxygen system inspections that are required by regulation. Carl R. Darnall Army Medical Center (CRDAMC) has a requirement to rent bulk liquid oxygen storage tanks and associated equipment for a base period from 1 Apr 2014 through 30 Sep 2014, and four (4) one-year option periods if exercised. The contractor retains title to equipment after the rental period. Equipment specifications: one (1) each three thousand (3,000) gallon medical-grade liquid oxygen bulk storage tank with appropriately one (1) each five hundred (500) gallon medical-grade liquid oxygen reserve liquid bulk oxygen tank and associated equipment (i.e., vaporizer and control equipment). Offeror's shall submit a copy of equipment brochure(s) that lists the name and address of the manufacturer, as well as, the specifications for the tank and associate equipment. The annual volume of medical-grade liquid oxygen used by CRDAMC is approximately 100,511 ccf per year. Army mission requirements call for CRDAMC to support a 260 bed medical facility. The 3,000 gallon bulk storage tank supports the mission requirement. Applicable Regulations & Standards The following is a list of some of the regulations and standards that are applicable to this contract. The list is not comprehensive, and the contractor is responsible for ensuring that all products, equipment and services provided under the contract are in compliance with applicable Federal, state, and local regulations. If applicable, the editions in effect as of the date of this solicitation are listed. Contractor is responsible for remaining compliant with any future revisions that are effective at the time of contract performance. Title 21, Code of Federal Regulations, Parts 210 & 211 - CGMP regulations for supplying medical grade oxygen. 29 CFR 1910.104 Applies to the installation of bulk oxygen systems on industrial and institutional consumer premises 49 CFR - Transportation Federal Food, Drug, and Cosmetic Act NFPA 50: Standard for Bulk Oxygen Systems at Consumer Sites, 2001 Edition NFPA 99: Standard for Health Care Facilities, 2002 Edition U.S.P. 23 Density data and volume measurement equivalents published in Compressed Gas Association Pamphlet No. P-6, titled quote mark Standard Density Data, Atmospheric Gases and Hydrogen, quote mark shall be used when necessary to convert measurement of gases from one form to another. For example, the following conversion factor shall apply for conversion from gallons to cubic feet. Calculation based on data in CGA P-6 1 ft3 liquid 02 = 860.6 ft3 gas (Table 1) 1 ft3 liquid 02 = 7.48052 gal (Table 2) Therefore: 7.48052 gal (1 ft3) liquid 02 = 860.6 ft3 gas 1 gal liquid 02 = 860.6 ft3 gas 7.48052 1 gal liquid 02 = 115.05 ft3 gas or more commonly quoted as 1 gal liquid 02 = 115.1 ft3 gas 1. Technical Acceptability: In order for a proposal to be deemed technically acceptable, the offeror must provide and/or substantiate the following requirements in their offer: a. Explain how your company will meet the above salient characteristics as specified in the solicitation; (Note) statements that the offorer understands, can or will comply with all specifications, or statements paraphrasing the specifications or parts thereof, or phrases such as standard procedures will be used or well known techniques will be used - will be considered insufficient and determined technically unacceptable. b. The provision 52.212-3, Offeror Representations and Certifications - Commercial Items, applies to this acquisition and offerors shall include a completed copy of the provision with its offer, OR indicate in the proposal that the offeror's applicable Reps and Certs are up to date in the System for Award Management (SAM) system at https://www.sam.gov. 2. Past Performance: The Government reserves the right to utilize internal and external resources to research offerors' past performance history, including, but not limited to the Excluded Parties Listing System (EPLS), the Past Performance Information Retrieval System (PPIRS), and the Department of Defense Termination List. 3. Price: The Offeror shall submit pricing in accordance with the attached Pricing Schedule, to be considered for an award, the RFP must be complete: the pricing on all line items must be completely filled in, to include; unit price and total price for each line item; the price for Bulk Liquid Oxygen per 100 SCF - for each option period; pricing for each option year, and a grand proposal total of the base period and all options. FAR Provision 52.212-2 applies to this acquisition. The Government anticipates the award of a FFP contract resulting from the solicitation to the responsible offeror whose offer, conforming to the solicitation, will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Technical Acceptability: In order for a proposal to be deemed technically acceptable, the offeror must provide and/or substantiate the following requirements in their offer: a.Statements that the offorer understands, can or will comply with all specifications, or statements paraphrasing the specifications or parts thereof, or phrases such as standard procedures will be used, or well-known techniques will be used - will be considered insufficient and determined technically unacceptable. Technical Acceptability will review the contractors plan for installation tanks and delivering of gas. Proposals not meeting the technical requirements in the solicitation will be determined technical unacceptable and will not be considered of award. The following ratings will be used: Acceptable Rating - Based on Southern Regional Contracting Office's market research, the proposal is sufficient to maintain the government's need. Unacceptable Rating - Based on Southern Regional Contracting Office's market research, the proposal is not sufficient to maintain the government's need. As the proposed items do not meet all salient characteristics. b.The provision 52.212-3, Offeror Representations and Certifications - Commercial Items, applies to this acquisition and offerors shall include a completed copy of the provision with its offer, OR indicate in the proposal that the offeror's applicable Reps and Certs are up to date in the System for Award Management (SAM) system at https://www.sam.gov. Offerors without Representations and Certifications or without a valid Representations and Certifications date will not be considered of award. 2. Past Performance: The Government reserves the right to utilize internal and external resources to research offerors' past performance history, including, but not limited to the Excluded Parties Listing System (EPLS), the Past Performance Information Retrieval System (PPIRS), and the Department of Defense Termination List. Positive Past Performance will be considered acceptable. Offerors with no Past Performance will receive a neutral rating. Negative Past Performance will be determined unacceptable and will not be considered of award. 3. Price: Offeror shall submit pricing in accordance with the attached Pricing Schedule, including pricing for all option years, to be considered for an award, pricing must be completely filled in, to include; unit price and total price for each line item; the price for Bulk Liquid Oxygen per 100 SCF - for each option period; pricing for each option year, and a grand proposal total of the base period and all options. Round all prices to two digits beyond decimal point; example $0.42. 4. Evaluation Process: All proposals will be evaluated on Technical Acceptability and Past Performance. Technical Acceptability and Past Performance, when combined, are equal to price. The award decision will be based on the lowest technically acceptable proposal. Award will be all or none basis. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor not less than 15 calendar days prior to the contract expiration date. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
