- Amendment 0003 *** Notice: Amendment 0003 is posted for this solicitation. This amendment establishes the Proposal due date for 11 February 2014, 10:30 a.m. CST, as instructed in the amended FAR 52.212-1 Addendum, along with other changes to the solicitation spelled out in the amendment. As a reminder, please be sure to submit ALL AMENDMENTS TO THIS SOLICITATION with your proposal. **Special Notice*** The deadline for receipt of proposals has been changed to indefinite. The amendment with the new deadline will be posted later today. Should you have any questions or concerns, please contact Mr. Jorge Canavati at 210-671-5145 or via email jorge.canavati@us.af.mil. ********************* This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Proposal (RFP) in accordance with FAR Parts 12, 13, and 15 utilizing the test procedures in FAR subpart 13.5 and the reference number is FA3047-13-R-0026. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition circular 2005-72 and DFARS change notice 20131231. This acquisition is set aside 100% for Small Business, NAICS Code 532210 with a size code of $35.5M. The contract line items numbers and items, quantities and units of measure pertaining to this requirement are identified on the Schedule B. The scope of work to be acquired is set forth in the Performance Work Statement (PWS). The PWS and Schedule B are available for download via www.fbo.gov. The contractor shall supply all personnel, equipment, tools, materials, supervision, and all other items or services necessary for Washer/Dryer Maintenance, Repair and Leasing and Clothes Dryer Exhaust Duct Cleaning service as defined in the PWS. The contract shall have a 4-year period of performance consisting of the base year plus three option years. Provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition along with the Addenda identified in in the solicitation. Provision 52.212-2, Evaluation-Commercial Items will be used, specific criteria are listed in the solicitation. Offerors are highly encouraged to complete all Representations and Certifications electronically at the following website: https://sam.gov. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisitionto include Addendum 52.212-4 details of which are included in the solicitation. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes Or Executive Orders - commercial Items, applies to this acquisition details of which are included in the solicitation.
