Description

The Department of Veterans Affairs, Network Contracting Office 23 located at 708 South Third Street, Suite 200E Minneapolis, MN 55415 is issuing a solicitation for The Black Hills VA Health Care System located at The Fort Meade Campus, 113 Comanche RD, Fort Meade, SD 57741 and Hot Springs Campus, 500 North 5TH ST, Hot Springs, SD 57747 for a contract to provide onsite Ophthalmology Services. The Ophthalmology Services are for the physician component only. The contract period will be for one (1) base year. It is anticipated that commencement of performance will be April 1, 2014 The contract will also include four (4), one (1) year option periods to be exercised at the discretion of the Government. NAICS Code 621111 applies with a size standard of $10 million. This notice does not commit the government to contract for any service. Please review and follow the ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS beginning on page 62-63 of the solicitation Please Note: All required solicitation documents shall be in the following software and size formats. Do not send as an executable or.exe file. Email size shall not exceed three megabytes. Any corrupted file or media containing a virus could result in the offer not being considered for award. Software Compatibility: use a word processing and spreadsheet program compatible with Microsoft ® Word 2007 ® and Microsoft ® Excel 2007 ®. PDF is Adobe ® Portable Document Format. All questions must be emailed to Contract Officer, Shea Colby @ shea.colby@va.gov. The question period will end COB 2/18/2014. A single amendment answering all questions will be posted to FBO.
