This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA259-14-Q-0241, issued as a request for quotation (RFQ). The items must be brand name or equal. If specifications are not the brand name listed, the vendor must submit documentation stating why their product is considered equal. The kits must include: The Hustead Winged 18 Ga. epidural needle 25 Ga. X 1.5 in hypodermic needle 3mL plastic Luer lock syringe 5mL glass Luer lock metal tip 20mL plastic Luer lock syringe 20 Ga. closed tip catheter Lidocaine both the 1% 5mL skin wheel and 1.5% with Epi test dose sodium chloride 10% for the diluent two type of drapes the blue non-fenestrated and the clear plastic 4 in fenestrated prep iodine solution prep sponges prep Perifix 0.2 µm filter prep catheter support pad needle block foam with holder filter straw paper towel 3ply 4in x 4in gauze sponge drug identification label set The anticipated contract is for a 1 year base plus 4 option years. The medical facility will need 170 cases (10 kits per case) per year. The vendor shall deliver 14 cases per month which will be scheduled by a VA employee. Delivery is to: VA Health Care System 500 Foothill Drive Salt Lake City, UT 84148-0001 Items will be FOB Destination. Items must be delivered within 30 Days ARO. The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. Offerors must be registered in SAM (www.sam.gov) to be eligible to receive Purchase Order award. This acquisition will be evaluated by LPTA (price and technical acceptability). The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. The following clauses are incorporated by reference: 52.222-50, 52.204-10, 52.209-6, 52.219-4, 52.219-28, 52.233-3, 52.233-4, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-37, 52.223-18, 52.225-1, 52.225-13, 52.232-33. 52.211-6 BRAND NAME OR EQUAL (AUG 1999) (a) If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. (b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must- (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by- (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. (c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation. 52.232-19 AVAILABILITY OF FUNDS FOR THE NEXT FISCAL YEAR (APR 1984) Funds are not presently available for performance under this contract beyond 03-04-2015. The Government's obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise for performance under this contract beyond 03-04-2015, until funds are made available to the Contracting Officer for performance and until the Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer. 52.217-6 OPTION FOR INCREASED QUANTITY (MAR 1989) The Government may increase the quantity of supplies called for in the Schedule at the unit price specified. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days. Delivery of the added items shall continue at the same rate as the like items called for under the contract, unless the parties otherwise agree. 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years. Offers are due at 12:00PM MST on 02/13/2014.
