Description
Synopsis: This is a sources sought notice only; it is not a request for competitive proposals. The Department of Veterans Affairs, Orlando VA Medical Center in Orlando, Florida is conducting market research to identify potential sources for a contract to furnish onsite and offsite eye exams and eyeglasses to those veterans and their beneficiaries that require them and are eligible for assistance. Facilities requiring onsite services are: Orlando VA Medical Center (both Lake Baldwin facility and, when available, the new Lake Nona facility), Viera OPC and Daytona OPC. Facilities requiring offsite services are: Orlando VA Medical Center (both Lake Baldwin facility and, when available, the new Lake Nona facility), Viera Outpatient Clinic, Daytona Beach Outpatient Clinic, Kissimmee OPC and Leesburg OPC. Services for eye examinations and subsequent prescription eyeglasses shall be provided at the awardee's location (Offsite services). Services to provide prescription eyeglasses only shall be provided by The awardee at the VA's location (Onsite services). The VA will provide an appropriate space for optical eyewear dispensary for onsite services. Room will be available during normal business hours 8:00 am to 4:30 pm. The awardee shall furnish employee desk & chairs, telephone, computer and fax equipment. The awardee shall provide all necessary equipment, materials and supplies required by optical technician(s) to perform services. Contractor furnished equipment shall be maintained, repaired or replaced as necessary, at no cost to the VA, when failure or malfunction is due to normal usage or due to the fault of The awardee. Contractor on-site services shall be required upon award to sign a Department of Veterans Affairs-Veteran Canteen Service (VCS) Concessionaire Agreement. Offsite services shall include providing eye examinations and eyeglasses. Contractor shall provide all necessary materials, labor, and space necessary to provide these services and products in accordance with specifications. Offsite contractor facilities shall be located within close proximity to the above mentioned locations. Optical Services shall include: Eye glasses laboratory, Optometrist or Ophthalmologists who possess license to practice in the state of Florida. The awardee shall accept prescriptions authorized by Prosthetics for fabrication of eyeglasses to eligible beneficiaries. In addition, Contractor shall perform eye exams to those patients referred by the Medical Center in strict conformance of the specifications under the Statement of Work and comply with the time frame established by VA directives. The awardee's facility or facilities shall be accessible and convenient for wheelchair patients and other handicapped or disabled Veterans in accordance with the American Disabilities Act. The awardee shall have an exam lane that is wheelchair accessible to preclude the need for transfers. There shall be adequate parking convenient to all VA patients to include handicapped parking spaces. The facility shall have computer(s) with the following capabilities to interface with the VA's medical record system: WIN XP, 128MB Ram and DSL Broadband Cable. The anticipated period of performance is from October 1, 2014 through September 30, 2015 with one option year from October 1, 2015 through September 30, 2016. Interested parties shall provide the following information/documentation for Government Evaluation: Points of Contact with names and current telephone numbers for Past Performance verification, Time (Years/Months) in business on same contract services, Company profile, including the name of the business, complete contact information, business size and socioeconomic category, such as: Service Disable Veteran Owned Small Business, Veteran Owned Small Business, Women-Owned Small Business, Hubzone Small Business, 8 (a) Small Business, must be registered in the official U.S. Government system that consolidated the capabilities of CCR/FedReg, ORCA, and EPLS. System for Award Management (SAM). Interested parties are reminded that to qualify as a Small Business under any of the above socioeconomic categories, in the event a set aside is done for any of these small business categories; Services (except construction). At least 50% of the cost of contract performance incurred for personnel shall be expended for employees of the concern (FAR 52.219-14-Limitations on Subcontracting). Interested parties shall be prepared to demonstrate how this requirement will be met, as meeting this requirement will be evaluated in determining the successful offeror once a solicitation is issued. Small business concerns should not claim eligibility for any given set-aside if they cannot meet the cost of personnel requirement for that set-aside. Due date for submission of company capabilities and experience in same or similar contracts is Friday, February 14, 2014 at 2:00 p.m. This Sources Sought Notice is to assist the VA in identifying an appropriate acquisition strategy only. A solicitation package is not available at this time, but a Statement of Work can be provided by electronic mail (e-mail) request ONLY. Once a solicitation is issued, the same will be announced/posted in this website (Federal Business Opportunities-FBO) with sufficient time for all interested parties to respond accordingly. Interested parties shall respond to the solicitation, addressing each of the evaluation factors and/or Statement of Work; in order to be considered for award. A determination by the Contracting Officer not to compete this action based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. This synopsis is not to be construed as a commitment by the Government and no contract will be awarded from this announcement.
