****Request for information only**** FBO ID# VA243-14-I-0456 Response date (Deadline): FEBRUARY 19, 2014 Sources Sought: Artificial Limbs & Related Services for VISN3 This request for information (RFI) is posted for market research purposes ONLY. It is not a solicitation. Statement of need: The Department of Veterans Affairs (VA) NY/NJ Veterans Healthcare Network VISN 3 has an ongoing program to furnish artificial limbs and related services to those veterans that require them and are eligible for assistance. Product or service description: "Artificial limbs, to include: "below knee and above knee artificial legs, "and below elbow and above elbow artificial limbs; "terminal devices; "complex knee and elbow systems; "myo-electric systems; "numerous socket and suspension systems; "Use of high technology and lightweight materials; "various manufacturing processes. VISN3 Sites / Local Geographic Area: Local Contractor: is defined as having a legitimate facilty/workplace within 100 miles of any of the sites listed below. It is of critical importance that we provide the maximum feasible number of local contractors in order to avoid delays in timeliness of service, and subjecting our veterans to factors such as unnecessary travel, and insufficient certified prosthetist availability. The artificial limbs and related services are to be provided to the following facilities in VISN 3: "Bronx, NY 10468; "Brooklyn, NY 11209; "Castle Point, NY 12511; "East Orange, NJ 07018; "Lyons, NJ 07939; "Montrose, NY 10548; "New York, NY 10010; "Northport, NY 11768; "St. Albans, NY 11425 Capability or performance: All contractors shall have a full-time American Board Certification (ABC), or Board for Orthotist-Prosthetist Certification (BOC) certified prosthetist on staff at each of their locations in good standing. Additionally, any state licensing requirement must be met where mandated by law. The Contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic Team. The fitting and alignment of the following artificial limbs/services must be performed under the supervision of a certified prosthetist who has met the applicable educational and training requirements. Veteran beneficiaries artificial limbs wearers may be measured, fitted, and accept conditional delivery of their artificial limb at the contractor's facility. Final delivery and check out will be conducted at the VA facility, unless otherwise indicated by the VA Amputee Clinic Team. New technologies require proof of training. Only those prosthetists specifically trained or receiving training for the purpose of providing the item and that training is complete by delivery of the item, may provide the item and follow up service to the Veteran. The contractors' facilities shall be accredited by either the American Board for Certification in Orthotics, Prosthetics, and Pedorthics Inc (ABC), or the Accredited Facilities Program of the Board of Certification International (BOC). Type of Procurement: The responses to this Request for Information (RFI) will help determine what, if any, type of set-aside Contract Vehicle: The resulting contract vehicle from this process will be either a multi-award IDIQ or a multi-award BPA A primary goal of this solicitation is to provide our veterans with reasonable access to contractors across our geographic area. In order to ensure timeliness of service for our veterans (as recommended by OIG) we are looking to have as many possible contractors as possible under this contract vehicle so that our veterans can have ample resources across VISN 3. Response date (Deadline): FEBRUARY 19, 2014 For planning and scheduling purposes, to insure being included in our market research for this product, responses must be received no later than FEBRUARY 19, 2014.
