- This is a PRE-SOLICITATION ANNOUNCEMENT ONLY. The Orlando VA Medical Center/Orlando VA Healthcare System OVAMC (Orlando, FL) intends to make a single small business award for the provision of oral surgery services. The oral surgery services shall be provided at the contractor's place of business, not to exceed 25 miles driving distance of the Orlando VA Healthcare Center, located at 5201 Raymond Street, Orlando, FL 32803 as referenced by Google Maps on the Internet. The contractor shall be capable of providing all necessary care for oral surgery services in support of the Orlando VA Medical Center. This includes all necessary care, supplies, facilities, equipment and support services. 1. Services shall be provided Monday-Friday 8:00 a.m.to 4:30 p.m. excluding government holidays. 2. On-Call/Off-Duty Hours for Emergency /Post-Procedure Care: shall be Monday through Friday from 4:30 p.m. to 8:00 a.m., 24-hour coverage weekends and holidays. 3. Services shall be provided on the premises of the Maxillo-Facial Oral Surgery Practice. 4. Perform all Maxillo-Facial Oral Surgery procedures under the nomenclature of dental coding and medical coding. 5. Routine dental exams shall be scheduled within (14) calendar days. 6.Treatment shall be completed within (30) days of initial patient contact or per case by case report. 7. Urgent exams shall be scheduled within (2) business day. 8. All requests shall be completed within 30 days of the appointment date. 9. Providers shall be privileged and credentialed by the contractor to include a Focused Professional Practice Evaluation (FPPE) at 3-months and on-going Professional Practice Evaluations (OPPE) every six months thereafter; providers shall have a current valid Florida Dental License, graduated from an American Dental Association (ADA) accredited school obtaining a Doctor in Dental Surgery (DDS) or Doctor in Medical Dentistry (DMD). 10. Providers shall be an Oral Maxillo-Facial Surgery Specialist that has completed the minimum post graduate training required by the ADA in an accredited program and completed oral surgery training through a 4 or 6 year residency. 11. Contractor shall be able to provide proof of completion, qualifications and credentialing. The contract type is Indefinite Delivery Indefinite Quantity (IDIQ) price contract with firm-fixed unit prices. The contract shall consist of a six (6) month base period and two (2) one (1) year option periods for renewal. The solicitation will be made available on or about February 12, 2014 and will be solely distributed through the General Services Administration's Federal Business Opportunities website at www.fedbizopps.gov. Hard copies of the solicitation will not be available. This site provides downloading instructions. All future information regarding this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about the acquisition. The anticipated date for receipt of proposals is February 28, 2014. All responsible sources may submit an offer which will be considered.
