- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is: VA741-14-Q-0063, this solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. This requirement is unrestricted and only qualified offerors may submit bids. The associated North American Industrial Classification System (NAICS) code for this procurement is 491110 with a $7 Million size standard. The Central Plains Consolidated Patient Account Center is in need of the following items: Contract line item number 0001: PO Box (Large) for CPCPAC incoming mail. Mail Code - 10NB1-735 CPAC, 22, EA. The Central Plains Consolidated Patient Account Center is in need of Post Office Boxes to receive mail to support our work. We need a contract that will allow us receive mail for the different facilities that we work with on a daily basis. The mail needs to be received into individual boxes corresponding to the 22 different stations that we perform work for. Period of Performance: PO Boxes shall be rented for One (1) Year FOB Destination shall be Leavenworth, Kansas. Solicitation and Buy Attachments ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.204-10, 52.219-4, 52.219-28, 52.222-3, 52.222-21, 52.222-26, 52.232-35, 52.222-36, 52.222-37, 52.223-18, 52.225-13, 52.232-34.
