Attachment 2: Request for Sole Source Memo Format DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Activity: Department of Veterans Affairs, VISN 11, Ann Arbor Medical Center proposes to purchase Meijer Brand gift cards for incentives for two research studies (506-14-1-099-0029). 2.Nature and/or Description of the Action Being Processed: Brand name justification for purchase of Meijer brand gift cards for two research studies. The brand name "Meijer" was determined to be an appropriate incentive for the following reasons: 1) there are multiple Meijer stores conveniently located throughout the region and the Veteran study participants reside near Meijer locations; and 2) study participants can use the gift cards to purchase a variety of items, such as merchandise and gasoline, which would be useful to Veteran patients and 3) use of the Meijer brand gift cards has proven to be a successful incentive during past studies compared to Gas cards or other brand gift cards and attracted the most veterans to participate in our studies. 3.Description of Supplies/Services Required to Meet the Agency's Needs: $26,175 estimated. This procurement request is for 975 $25 Meijer gift cards and 180 $10 Meijer gift cards. The gift cards shall be used as research participant incentives in two VA HSR&D-funded research studies entitled, "Technology-Assisted Peer Support for Recently Hospitalized Depressed Veterans" and "Employment Support Needs of VA Primary Care Patients with Depression and Anxiety" as a way to help with recruitment and retention of study participants. The use of incentives is an empirically based standard practice that has been shown to reduce non-response bias and increase response rates. 4.Statutory Authority Permitting Other than Full and Open Competition: (x) (1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements per FAR 6.302-1; ( ) (2) Unusual and Compelling Urgency per FAR 6.302-2; ( ) (3) Industrial Mobilization, Engineering, Developmental or Research Capability or Expert Services per FAR 6.302-3; ( ) (4) International Agreement per FAR 6.302-4 ( ) (5) Authorized or Required by Statute FAR 6.302-5; ( ) (6) National Security per FAR 6.302-6; ( ) (7) Public Interest per FAR 6.302-7; 5.Demonstration that the Contractor's Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): The use of the Meijer brand gift cards has proven to be a successful incentive during past studies compared to just gas cards or other brand gift cards and Meijer cards attracted the most veterans to participate in our studies. 6.Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: This procurement will be competed on FBO via a Synopsis Solicitation. 7.Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable: $26,175 estimated.
