Description

(I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This acquisition is Set-Aside for Small Business Concerns. All Small Businesses submitting an offer must be registered on https://www.sam.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). (II) This solicitation is issued as a Request For Quote (RFQ). Submit written proposals on RFP Number VA246-14-Q-0398. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66 and the VA Acquisition Regulation (VAAR). (IV) This procurement is unrestricted. The associated NAICS code is 339112. The small business size standard is 500 employees. (V) This combined solicitation/synopsis is for purchase of the following commercial items: 6) The Asheville VA Medical center requires the following: 0001 Request vendor furnish all labor and material to remove old and install new Air Dryer at Medical Air System located in the old Boiler Room Building 47, VAMC Ashville. 7) The statement of work : Replace Air Dryer at Boiler Room Building 47 (Service) PROJECT #637-14-140S STATEMENT OF WORK (1)General. Special Requirement -- The Contractor will provide all transportation, material hardware and parts, tools, specialized equipment, labor, supervision and technical expertise to replace air dryer at medical air system located at old Boiler Room Building 47, VA Medical Center, Asheville NC. (2)Coordination. Maintenance and Operations Services will serve as the focal point and Contraction Office Representative (COR) for this contract. (2.1)Site Visits. Contractors must accomplish a site visit prior to submitting their proposal. Site visits must be pre-coordinated and prescheduled with the COR. Unannounced site visits will not be honored. The Federal Government will not incur any cost associated with site visits. (2.2)The Contractor must pre -coordinate work and if needed, associated outages with the COR. This work will be accomplished during regular work hours. The contractor will respond to Building 47, Room EB01 on the prescheduled date/time, and make contact with the COR. Contact must be made with the COR prior to accomplishing the starting any work. (2.3)The COR will be immediately notified whenever the replacement of the medical air dryer, or related activities interfere with normal VAMC operation. (3)Specific Requirements. The Contractors work shall include, but not be limited to: "Remove old and install new duplex air dryer with CO (Carbon Monoxide) and Dew Point monitor. "Replacement air dryer must equal to or greater than in quality and performance to Part #4017006941 / LDD075D-DRC Lifeline Dryer Duplex 75 CFM with CO and Dew Point Regulator. "Air Dryer and all components shall be installed by ASSE 6010 licensed installer. "New dryer must have two year parts and labor warranty. "Vendor will provide Medical Gas Certification upon Completion of installation. "NOTE: Work is being completed in a hospital environment. The Contractor must take all precautions to ensure all tools and materials are under continuous observation and accountability. At no time will tools be left unattended or unsecured. "When complete, notify the COR to inspect and accept all accomplished work. "Provide the COR all documentation and warranty information concerning installed components/equipment and work accomplished. "Ensure all invoicing documents are submitted in-accordance with the instructions provided in the formal contract. (4) Other Station Rules and Guidelines. The Contractor will adhere to the following station rules. 4.1. All work will be accomplished to industry standards and compliant with all current CODE requirements (Life Safety, NFPA, Electrical and Building). All Dept. of Veteran Affairs, Federal regulations and codes will be complied with. 4.2. This is a hospital environment. The Contractor and his personnel are expected to be appropriately dressed (preferably uniformed) and to conduct themselves professionally. 4.3. This is a federal facility as well as a medical center. As such, there is NO tobacco usage allowed inside any federal building or on the grounds except in designated areas. The COR will brief the Contractor as to designated smoking areas prior to commencement of the contract. Failure to follow these regulations can result in removal of the worker or Contractor from the facility and/or a US Magistrate's ticket and fine. 4.4. Parking will be in designated lots. Consult with the COR as to allowable parking and material drop-off. 4.5. Any work requiring a shutdown or utility outage will be requested in advance and scheduled by the COR prior to the work being accomplished. 4.6. The Contractor will coordinate his activities with the COR to identify any smoke or fire barriers that must be penetrated or disrupted as a result of this contract and will restore the fire barrier to its rating through the use of fire stopping. The Contractor and his personnel will be trained and certified to use and apply fire stopping prior to its application and at the Contractor's expense. 4.7. The Contractor will responsible for ensuring that all the employees and other personnel visiting the work site have and are using the appropriate Personal Protective Equipment (PPE) that addresses the particular hazards found at the site. The Contractor will be responsible for the upkeep, issue, and training for the PPE used and maintaining all required documentation 4.8. Since the incidents of September 11, 2001, the Federal Government has begun to implement new security measures to help protect federal facilities. The Contractor will provide to the COR a list of employees expected to work on this contract. This list will be on company letterhead and contain the employees' name, driver's license number, address and nationality. This list will be verified and signed by a corporate officer. These personnel are subject to a background check and fingerprinting to be eligible to receive a security badge. 4.9. Identification Badges will be picked up from Work Order Desk Room EB22 in Building 47. 8) The place of performance is the Charles George VAMC, 1100 Tunnel Rd., Asheville, NC 28805. 9) The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. 10) Evaluation procedures: The VAMC Asheville intends to award a Firm Fixed Price Contract to the responsible offeror submitting the lowest priced offer that conforms to the solicitation. Offerors may revise offers anytime during the specified period. At the end of the specified time period for receipt of offers, the responsible offeror submitting the lowest priced offer will be in line for award. IAW VAAR 812.302 Tailoring of Provisions and Clauses for the Acquisition of Commercial items, this contract will be awarded using a Lowest Price Technically Acceptable (LPTA) approach. Technical ability will be evaluated as either Acceptable/Unacceptable, Pass/Fail, or Go/No-Go. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. VAAR 852.203-70 Commercial Advertising (JAN 2008) The bidder or offeror agrees that if a contract is awarded to him/her, as a result of this solicitation, he/she will not advertise the award of the contract in his/her commercial advertising in such a manner as to state or imply that the Department of Veterans Affairs endorses a product, project or commercial line of endeavor. (End of Clause) VAAR 852.233-70 Protest Content/Alternative Dispute Resolution (JAN 2008) (a) Any protest filed by an interested party shall: (1) Include the name, address, fax number, and telephone number of the protester; (2) Identify the solicitation and/or contract number; (3) Include an original signed by the protester or the protester's representative and at least one copy; (4) Set forth a detailed statement of the legal and factual grounds of the protest, including a description of resulting prejudice to the protester, and provide copies of relevant documents; (5) Specifically request a ruling of the individual upon whom the protest is served; (6) State the form of relief requested; and (7) Provide all information establishing the timeliness of the protest. (b) Failure to comply with the above may result in dismissal of the protest without further consideration. (c) Bidders/offerors and contracting officers are encouraged to use alternative dispute resolution (ADR) procedures to resolve protests at any stage in the protest process. If ADR is used, the Department of Veterans Affairs will not furnish any documentation in an ADR proceeding beyond what is allowed by the Federal Acquisition Regulation. (End of Provision) VAAR 852.233-71 Alternate protest procedure (JAN 1998) As an alternative to filing a protest with the contracting officer, an interested party may file a protest with the Deputy Assistant Secretary for Acquisition and Materiel Management, Acquisition Administration Team, Department of Veterans Affairs, 810 Vermont Avenue, NW., Washington, DC 20420, or for solicitations issued by the Office of Construction and Facilities Management, the Director, Office of Construction and Facilities Management, 810 Vermont Avenue, NW., Washington, DC 20420. The protest will not be considered if the interested party has a protest on the same or similar issues pending with the contracting officer. ( End of provision) VAAR 852.246-70 Guarantee (JAN 2008) The contractor guarantees the equipment against defective material, workmanship and performance for a period of, said guarantee to run from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material that are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States, or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside of the continental United States. Cost of installation of replacement material and parts shall be borne by the contractor. (End of Clause) VAAR 852.246-71 Inspection (JAN 2008) Rejected goods will be held subject to contractors order for not more than 15 days, after which the rejected merchandise will be returned to the contractor's address at his/her risk and expense. Expenses incident to the examination and testing of materials or supplies that have been rejected will be charged to the contractor's account. (End of Clause) VAAR 852.252-70 Solicitation provisions or clauses incorporated by reference (JAN 2008) The following provisions or clauses incorporated by reference in this solicitation must be completed by the offeror or prospective contractor and submitted with the quotation or offer. Copies of these provisions or clauses are available on the Internet at the Web sites provided in the provision at FAR 52.252-1, Solicitation Provisions Incorporated by Reference, or the clause at FAR 52.252-2, Clauses Incorporated by Reference. Copies may also be obtained from the contracting officer. (End of Provision) The Service Contract Act of 1965 (SCA) is applicable to this procurement.
