STATEMENT OF NEED STANDARD FORM 1449 (for pricing) SPECIFICATIONS COMBINED SYNOPSIS/SOLICITATION PROJECT TITLE: RADIO FREQUENCY IDENTIFICATION (RADIO) TAGS FOR JUVENILE SALMON PROJECT LOCATION: PASCO, WA STATEMENT OF NEED: Contractor shall provide all labor, tools, equipment, materials and supplies necessary to provide the required radio tags, in accordance with the attached Statement of Need (SON) and specifications. SOLICITATION INSTRUCTIONS AND REQUIREMENTS: (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a Request For Quotation (RFQ) and has been assigned Solicitation Number AB-133F-14-RQ-0156. The Government intends to award a Firm-Fixed-Price contract based on its selection of the offer that represents the best value to the Government. (III) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72, effective 1 January 2014. (IV) This solicitation is being issued as a Total Small Business Set-Aside. The associated NAICS code is 334202, Radio and Television Broadcasting and Wireless Communication Equipment Manufacturing. The small business size standard is 750 employees. (V) For a list of commercial services and/or items being purchased for this requirement, refer to Attachment 1, STANDARD FORM 1449. (VI) For a description of the requirements associated with this acquisition, refer to Attachment 2, STATEMENT OF NEED and SPECIFICATIONS. (VII) PERIOD OF PERFORMANCE: Date of Contract Award through 5 April 2014. (VIII) FAR 52.212-1, INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS (JUL 2013) The offeror will submit a quote for the items referred to in Section (V) and listed in Attachment 1, STANDARD FORM 1449, and in Section (VI) and listed in Attachment 2, STATEMENT OF NEED and SPECIFICATIONS. NOTICE TO OFFERORS: Instructions for submitting quotations under this RFQ solicitation must be followed exactly as written. Offerors who fail to provide all specified information may be considered non-responsive. Quotes from offerors who have been deemed non-responsive may be excluded from further evaluation and rejected without further notification to the offeror. ***All vendors doing business with the Government are required to be registered with the System for Award Management (SAM), at www.sam.gov, which replaces the CCR and ORCA systems formerly used for this purpose. No award can be made unless the vendor is registered in SAM. *** In order to register in SAM and be eligible to receive an award from this acquisition office, all vendors must first have a Data Universal Numbering System (DUNS) number issued by Dun & Bradstreet, Inc. Vendors may obtain a DUNS number free of charge by visiting http://fedgov.dnb.com/webform or by calling the Dun & Bradstreet Customer Resource Center at (800) 333-0505. Offerors are further instructed to: (1) Submit quotes and all other requested documentation along with a completed STANDARD FORM 1449. (2) Submit quotes to the office identified in this solicitation, at or before the specified date and time. Electronically-sent quotes are preferable and may be emailed to Michael Drummond, Contract Specialist, at Michael.Drummond@noaa.gov or faxed to his attention at (206) 526-6025. (3) Provide, at a minimum, the following additional information: a. Name, Telephone Number, and Email Address of your designated Point-of-Contact. b. DUNS number for your company. (4) Respond to all evaluation factors listed in Section (IX) FAR 52.212-2, Evaluation - Commercial Items (JAN 1999), for this solicitation. (5) Formally acknowledge each amendment to this solicitation, if applicable, by completing the appropriate sections on the corresponding Standard Form 30, Amendment Of Solicitation/ Modification Of Contract (i.e., Block 8 and Blocks 15a through 15c). (IX) FAR 52.212-2, EVALUATION - COMMERCIAL ITEMS (JAN 1999) Offers will be evaluated based on the factors set forth in paragraph (a), and award will be made to the firm offering the best value to the Government. (1) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: A. Specifications: Items offered meet technical specifications stated in Attachment 2 to the solicitation. B. Past Performance: The Government reserves the right to assess the past performance of proposed contractors and will use its discretion to determine the sources of past performance information used in the evaluation (Note: The information may be obtained from references provided by the offeror, the agency's knowledge of contractor performance, other Government agencies or commercial entities, or past performance databases). The offeror shall provide the names, phone numbers, and email addresses of at least two (2) references for whom supplies of an identical or substantially similar nature were provided and who can attest to the offeror's responsiveness, quality of supply, and level of customer service within the last three (3) years (in the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance); and C. Price. Technical and past performance, when combined, are more important than price. Evaluation factors are listed in descending order of importance. (2) Options. The Government will evaluate offers for award purposes by adding the total price for all options, if any, to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (3) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of provision) (X) FAR 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS (NOV 2013) An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically via http://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision.* (b) (1) Annual Representations and Certifications. Any changes provided by the offeror in paragraph (b)(2) of this provision do not automatically change the representations and certifications posted on the SAM website. (2) The offeror has completed the annual representations and certifications electronically via the SAM website accessed through https://www.acquisition.gov. After reviewing the SAM database information, the offeror verifies by submission of this offer that the representation and certifications currently posted electronically at FAR 52.212-3, Offeror Representations and Certifications- Commercial Items, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard applicable to the NAICS code referenced for this solicitation), as of the date of this offer and are incorporated in this offer by reference (see FAR 4.1201), except for paragraphs _.[Offeror to identify the applicable paragraphs at (c) through (o) of this provision that the offeror has completed for the purposes of this solicitation only, if any.
