- STATEMENT OF WORK - COURIER SERVICES 1.A fully licensed courier company will provide delivery between the Wilkes-Barre Veterans Affairs Medical Center (WBVAMC), 1111 East End Boulevard, Wilkes-Barre, PA 18711 and various other locations throughout the state of Pennsylvania as listed in this document. 2.This agreement is effective March 31, 2014 - March 31, 2015. 3.The following training, insurance, licenses, certifications, etc. are required and proof must be presented with the offer. Contractor must update them as required and when additional drivers are added to perform services under this contract. Such documentation shall be forwarded to the COR upon five business days of receipt. 4.Courier personnel will be trained in the safe transport of infectious/ diagnostic specimens as required by federal, state and local law. 5.Courier will hold insurance in the amounts required under Pennsylvania law for the transport of property and, if applicable, passengers. 6.Courier will hold all required federal, state and local licenses and permits required for the transport of diagnostic/ biohazard specimens. 7.All drivers performing services under this contract must possess a valid driver's license for the type of vehicle operated as required by the State of Pennsylvania. 8.Vehicles used in the performance of this contract must be registered in the State of Pennsylvania and comply with applicable state mandates. 9.Courier personnel will be uniformed to include a photo ID badge worn above the waist when inside either the pick-up or delivery location. Offeror must submit a color photo indicative of a typical delivery vehicle and a uniformed courier. 10.Courier is responsible for maintaining a log of all pick-ups and deliveries. Logs will indicate the time and dates of pick-ups and deliveries, number of specimens/ parcels received or delivered, and the appropriate signatures. Logs shall be available for review at any time during the contract. 11.Deliveries performed without any of the above, will not be charged to WBVAMC, and will not be paid by WBVAMC. Late deliveries will be invoiced and paid at a 15% discount for each thirty-minute period of delay unless otherwise specified. More than one unsubstantiated late delivery greater than thirty minutes in thirty day period will be cause for Termination for Default. 12.Couriers must abide by FAR 52.219-14, Limitations of subcontracting, and offerors shall submit a subcontracting plan in accordance with FAR 52.219-9 along with quotes when applicable. 13.The following is a list of services that will be needed: A. Allentown VA Outpatient Clinic to Wilkes-Barre VAMC, Wilkes-Barre, PA 1. A fully-licensed courier company will provide delivery from the Allentown VA Outpatient Clinic (AOPC), 3110 Hamilton Boulevard, Allentown, PA 18103 to WBVAMC, 1111 East End Boulevard, Wilkes-Barre, PA 18711 Monday through Friday with exception of federal holidays. 2. Courier will arrive daily at the Allentown Clinic not earlier than 1:00PM and report to the laboratory where they will receive diagnostic specimens inside approved (triple packaged and labeled) containers. Size of this pick-up is approximately 15" X 19" X 22". 3.Courier will deliver the specimen containers not later than 2:30PM to WBVAMC Laboratory, third floor, Room A3-36. Courier will receive empty containers from the previous day's delivery, which will be returned to the Allentown Clinic on the next scheduled pick-up. B. Sayre VA Outpatient Clinic to Wilkes-Barre VAMC, Wilkes-Barre, PA 1. A fully-licensed courier company will provide delivery from the SayreVA Outpatient Clinic (SOPC), 1537 Elmira Street, Sayre, PA 18840 to WBVAMC, 1111 East End Boulevard, Wilkes-Barre, PA 18711 Monday through Friday with exception of federal holidays. 2. Courier will arrive daily at the Sayre Clinic not earlier than 2:15PM and report to the laboratory where they will receive diagnostic specimens inside approved (triple packaged and labeled) containers. Size of this pick-up is approximately 15" X 19" X 22". 3.Courier will deliver the specimen containers not later than 4:30PM to WBVAMC Laboratory, third floor, Room A3-36. Courier will receive empty containers from the previous day's delivery, which will be returned to the Allentown Clinic on the next scheduled pick-up. C. Bangor VA Outpatient Clinic to Wilkes-Barre VAMC, Wilkes-Barre, PA 1. A fully-licensed company will provide delivery from the Bangor VA Outpatient Clinic, 701 Slate Belt Boulevard, Bangor, PA 18013 to WBVAMC, 1111 East End Boulevard, Wilkes-Barre, PA 18711 Monday through Friday with exception of federal holidays. Once per week the courier will provide delivery from WBVAMC to the Bangor and Tobyhanna VA Outpatient Clinics. 2.Courier will arrive daily at the Bangor Clinic not earlier than 10:45AM to receive diagnostic specimens inside an approved (triple-packaged and labeled) container, pharmaceutical prescriptions and/ or medications inside a secure container and sealed inter-office mail envelopes. Total size of this pick-up is approximately 15" X 19" X 22". Approximately twice per month the courier may receive one or two miscellaneous items (not to exceed 30lbs) for transport. Inter-office mail and documents must be transported in a secure manner, i.e. in a locked container or equivalent to be provided by the Courier, to ensure privacy and immobility during transport. 3.This run will include a stop at the Tobyhanna Outpatient Clinic, 11 Hap Arnold Blvd., Building 220, Tobyhanna, PA 18466 two days each week, Wednesday, and Thursday, not earlier than 11:30AM to receive items identified in Paragraph 2 for this location. 4.Courier will deliver the specimen containers not later than 1:30PM to WBVAMC Laboratory, third floor, Room A3-36. Courier will receive empty containers from the previous day's delivery, which will be returned to the Bangor and Tobyhanna Clinics on the next scheduled pick-up. Courier will proceed to deliver the pharmaceutical container to the Outpatient Pharmacy Back Door, Basement Floor, Room CB-84, and the remaining packages/ materials to the Mail Room, Basement Floor, Room CB-10. 5.Once per week, on a day to be determined between the parties, courier will arrive at the WBVAMC Warehouse to pick-up items for delivery to the Tobyhanna and Bangor Clinics. Items will be picked up in the Warehouse in the morning prior to pick-up of items at the CBOC. These items include secure pharmaceutical and empty specimen containers, office supplies and inter-office mail envelopes in one or two containers, each approximately 12" x 18" x 10", and occasionally, one or two miscellaneous items (not to exceed 30lbs). These items will be delivered to the Bangor and Tobyhanna Clinics on the same day they are picked up. 6.during certain weeks. Offerors should base their rates on the daily runs. D. Williamsport VA Outpatient Clinic to Wilkes-Barre VAMC, Wilkes-Barre, PA 1.A fully-licensed courier company will provide delivery from the Williamsport VA Outpatient Clinic, 1705 Warren Avenue, Wenner Building of Divine Providence Hospital Campus, Suite 304, Williamsport, PA 17701 to WBVAMC, 1111 East End Boulevard, Wilkes-Barre, PA 18711 Monday through Friday with exception of federal holidays. Once per week the courier will provide delivery from WBVAMC to the Williamsport VA Outpatient Clinic. 2.Courier will arrive daily at the Williamsport Clinic not earlier than 12:00 Noon to receive diagnostic specimens inside an approved (triple-packaged and labeled) container, pharmaceutical prescriptions and/ or medications inside a secure container and sealed inter-office mail envelopes. Total size of this pick-up is approximately 15" X 19" X 22". Approximately twice per month the courier may receive one or two miscellaneous items (not to exceed 30lbs) for transport. Inter-office mail and documents must be transported in a secure manner, i.e. in a locked container or equivalent to be provided by the Courier, to ensure privacy and immobility during transport. 3.Courier will deliver the specimen containers not later than 2:00PM to WBVAMC Laboratory, third floor, Room A3-36. Courier will receive empty containers from the previous day's delivery, which will be returned to the Williamsport Clinic on the next scheduled pick-up. Courier will proceed to deliver the pharmaceutical container to the Outpatient Pharmacy Back Door, Basement Floor, Room CB-84, and the remaining packages/ materials to the Mail Room, Basement Floor, Room CB-10. 4.Once per week, on a day to be determined between the parties, courier will arrive at the WBVAMC Warehouse to pick-up items for delivery to the Williamsport Clinic. Offerors should base their rates on the daily runs. E. Columbia County VA Outpatient Clinic to Wilkes-Barre VAMC, Wilkes-Barre, PA 1.A fully-licensed company will provide delivery from the Columbia County VA Outpatient Clinic, 301 West third Street, Berwick, PA 18603 to WBVAMC, 1111 East End Boulevard, Wilkes-Barre, PA 18711 Monday through Friday with exception of federal holidays. Once per week the courier will provide delivery from WBVAMC to the Williamsport VA Outpatient Clinic. 2.Courier will arrive daily at the Columbia County Clinic not earlier than 2:30PM to receive diagnostic specimens inside an approved (triple-packaged and labeled) container, pharmaceutical prescriptions and/ or medications inside a secure container and sealed inter-office mail envelopes. Total size of this pick-up is approximately 15" X 19" X 22". Approximately twice per month the courier may receive one or two miscellaneous items (not to exceed 30lbs) for transport. Inter-office mail and documents must be transported in a secure manner, i.e. in a locked container or equivalent to be provided by the Courier, to ensure privacy and immobility during transport. 3.Courier will deliver the specimen containers not later than 4:00PM to WBVAMC Laboratory, third floor, Room A3-36. Courier will receive empty containers from the previous day's delivery, which will be returned to the Columbia County Clinic on the next scheduled pick-up. Courier will proceed to deliver the pharmaceutical container to the Outpatient Pharmacy Back Door, Basement Floor, Room CB-84, and the remaining packages/ materials to the Mail Room, Basement Floor, Room CB-10. 4.Once per week, on a day to be determined between the parties, courier will arrive at the WBVAMC Warehouse to pick-up items for delivery to the Columbia County Clinic. Offerors should base their rates on the daily runs. F. Wilkes-Barre VAMC, Wilkes-Barre, PA to Philadelphia VAMC 1. A fully-licensed courier company will provide delivery from the Wilkes-Barre VAMC, 1111 East End Boulevard, Wilkes-Barre, PA 18711 to the Philadelphia VAMC, 3900 Woodland Avenue, Philadelphia, PA 19104 Monday through Friday with exception of federal holidays. 2.Courier will arrive daily at WBVAMC Laboratory, Third Floor, Room A3-36, not earlier than 5:00PM to receive diagnostic specimens inside approved (triple-packaged and labeled) containers. Size of this pick-up is approximately five containers, each 15" x 19" x 22". 3.Courier will deliver the specimen containers not later than 9:00PM to Philadelphia VAMC Laboratory, Third Floor, Room 3B-138. Courier will receive empty containers from the previous day's delivery, which will be returned to WBVAMC on the next scheduled pick-up. G. Blood Delivery, Routine: American Red Cross, Hanover Township, PA to WBVAMC 1. Upon notification by a WBVAMC Medical Technologist (lab), Courier will pick up blood products in a secured container from the American Red Cross, 29 New commerce Boulevard, Ashley, PA 18706 and deliver to the Emergency Room Reception Area, WBVAMC. 2.Routine orders number approximately 15 - 20 per month, and are placed by phone, typically between 8:00AM and 10:00PM, 7 days per week. Orders must be delivered to WBVAMC within 90 minutes of the order to preserve the contents. 3.Deliveries made outside the timeframes specified above will be invoiced and paid at 50% of the agreed-upon price unless late deliveries are excused in writing by the Contracting Officer Representative (COR). More than two unexcused late deliveries in a two-month period will result in Termination for Default. H. Blood Delivery, Immediate: American Red Cross, Hanover Township, PA to WBVAMC 1. Upon notification by a WBVAMC Medical Technologist (lab), courier will pick up blood products in a secured container from the American Red Cross, 29 New Commerce Boulevard, Ashley, PA 18706 and deliver to the Emergency Room Reception Area, WBVAMC. 2.STAT orders, which must be delivered to WBVAMC within 45 minutes of the order, number approximately 8/ month, are placed by phone, and can be ordered at any time of the day, 7 days/ week. Deliveries made outside the timeframes specified above will be invoiced and paid at 50% of the agreed-upon price unless late deliveries are excused in writing by the Contracting Officer Representative (COR). More than two unexcused late deliveries in a two-month period will result in Termination for Default. I. Miscellaneous local deliveries between WBVAMC and other facilities within the general Wyoming Valley Area 1. A fully-licensed courier company will provide delivery. 2.Notification and details will be provided by telephone by a WBVAMC staff member. These calls can be placed 24 hours/ day, 7 days/ week, though they generally occur during the day and evening hours. 3.This function includes both the transport of eligible beneficiaries, with or without parcels/ luggage, as well as medical items including, film, reports and products. All documents must be transported in a secure manner, i.e. in a locked container or equivalent to be provided by the Courier, to ensure privacy and immobility during transport. 4.Mileage will be determined by www.mapquest.com with the WBVAMC address, 1111 East End Boulevard, Wilkes-Barre, PA 18711, as either the origin or destination point. Total one-way billable miles will be rounded up to the next full mile. 5.Monthly invoice will display the date, time, full address of the origin/ destination, nature of trip, and the miles for each trip. J.Miscellaneous local deliveries between WBVAMC and other facilities outside the Wyoming Valley Area 1. A fully-licensed courier company will provide delivery. 2.Notification and details will be provided by telephone by a WBVAMC staff member. Courier will pick up parcels at the WBVAMC loading dock. 3.This function includes both the transport of a larger amount of office and medical supplies, for which advance notice of two business days will be provided. A dedicated or direct delivery run is not required or expected; delivery must be made within 24 hours of pick-up. All documents must be transported in a secure manner, i.e. in a locked container or equivalent to be provided by the courier to ensure privacy and immobility during transport. 4.Mileage will be determined by www.mapquest.com with the WBVAMC address, 1111 East End Boulevard, Wilkes-Barre, PA 18711, as either the origin or destination point. Total one-way billable miles will be rounded up to the next full mile. 5.Monthly invoice will display the date, time, full address of the origin/ destination, nature of trip, and the miles for each trip. Billing: 1. Billing will be in arrears on a monthly basis. Invoices will be sent to the Contracting Officer at WBVAMC, 1111 East End Blvd., Wilkes-Barre, PA 18711. Economic Price Adjustment This contract is subject to an economic price adjustment to account for changes in regular unleaded gasoline prices. Pricing data will be taken from the following website: http://www.eia.doe.gov/oil gas/petroleum/data publications/wrap/mogas home page. html The prices listed for the U.S. East Coast Central Atlantic region will be used, and pricing will be monitored and recorded by the contractor and the contracting officer. As gasoline prices change, per mile rate will adjust accordingly. Gasoline prices are reported on the website every week, either Monday or Tuesday. The EPA will look at the average price of regular unleaded gasoline each month as compared to the base rate. The base rate will be the first gasoline rate reported on or after the date of award. For every 10% increase or decrease in the price of gasoline, the per mile base rate (the original price, not the subsequent, modified prices) will adjust 1.5%. Assuming gasoline accounts for roughly 15% of a contractor's expenses, a 10% change in fuel prices will cause the contract price to adjust by 1.5%. Prices will be monitored and recorded every Monday by the contractor. If following the last report for each month, the contractor notices that a price adjustment is necessary, either an increase or a decrease, they are required to notify the contracting officer representative by close of business (4:30PM) that Friday. They should provide supporting documentation, including a print out of the prices as listed on the aforementioned website. The actual price change will be incorporated into the contract as a modification, and the new prices will be effective the following Monday. If the last Monday in the month is Nov 26, the contractor has until Friday, Nov 30 to notify the contracting officer representative, and the new prices will be effective Monday, Dec 3.
