- AMENDED TO CHANGE COVERAGE TIME PERIOD OF INSTRUMENTS WITH SERIAL#'S 2560000137 AND 2102000263 BOTH SHOULD BE FOR 12 MONTHS, THERE ARE NO OTHER CHANGES TO THIS NOTICE. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is NICHD-14-011a and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-72. The North American Industry Classification (NAICS) Code is 811219 and the business size standard is $19M. However, this solicitation is not set aside for small business. This acquisition is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) intend to procure a Service/Support Maintenance Agreement for the following Carl Zeiss Microscopy manufactured instruments: EQUIPMENT SYSTEM SERIAL NUMBER NUMBER MONTHS OF COVERAGE Note: Coverage dates of the following instruments will be for one full calendar year LSM 510 UV/W ONE MICROSCOPE 851187 12 MONTHS LSM 510 UV/W ONE MICROSCOPE 2101000205 12 MONTHS VIS/NLO SCAN MODULE LSM 510 2433000118 12 MONTHS SCANHEAD LSM 710 2504000301 12 MONTHS SCANHEAD LSM 710 2502000484 12 MONTHS Note: Coverage date of the following instrument is 4/1/14 through 12/31/14 SCANHEAD LSM 710 2504000417 9 MONTHS ILLUMINATION MODULE PAL-M +SIM 2560000137 12 MONTHS Note: Coverage date of the following instrument is 9/1/14 through 12/31/14 LSM 510 UV/W ONE MICROSCOPE 851035 4 MONTHS SCAN MODULE VIS/405 META 2102000263 12 MONTHS Note: All of the above instruments listed are located on/within the NIH Bethesda, MD campus. Service Coverage to include but not limited to: Scheduled and Emergency service performed between the hours of 8:00am and 5:00pm, Monday thru Friday, excluding holidays All travel charges w/in the continental United States One (1) preventative maintenance inspection (where applicable) per Service Agreement year All covered replacement parts Applications Support/Training Software updates when available Factory trained Field Service Representatives Please see Statement of Work and Technical Evaluation Attachments for additional information and requirements. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-1 Instructions to Offerors - Commercial Items; FAR Clause 52.212-2, Evaluation - Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation for Simplified Acquisitions applies to this acquisition. The offeror must include their Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Interested vendors with the capability of providing, at a minimum fifty-one percent (51%) of the government needed services specified in this synopsis should submit a copy of their quotation to the below address or via email to robinsti@mail.nih.gov. Offers must also be accompanied by descriptive literature, warranties and/or other information that demonstrates that the offer meets all of the foregoing requirements. Quotations will be due fifteen (15) calendar days from the publication date of this synopsis or by March 6, 2014 by 12pm (noon) EST. via email or postal mail or other mailing services. The quotation must reference "Solicitation number" NICHD-14-011a. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency.
