Description

SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is conducting a market survey to identify potential sources to provide professional grief surveyor services. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources of providing grief interviewers to contact next of kin by telephone and/or mailing of questionnaire. Surveyor will collect the data provided and transcribe answers into a national database. The provision of services includes skilled technical personnel at the Lebanon VA Medical Center location. The North American Industry Classification System Code (NAICS) is 561422 and the small business size standard is $14.0 million. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of grief surveyor skills. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.Company Name, Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 561422 and the small business size standard is $14.0 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3.Provide a summary of experience related to palliative surveys or questionnaires. 4.Provide a description of experience compiling and reporting collected information. 5.If company holds a Federal Supply Schedule (FSS) Contract, please list the GSA Contract Number and relevant SINS. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
