- SF30 Amendment (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The solicitation is issued as a Request For Quotation (RFQ) AG-344V-S-14-0006, Caney Creek FRP Bridge. Submit written offers only, oral offers will not be accepted. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 72. Full text clauses and provisions may be accessed electronically at these address(es): Federal Acquisition Regulations - http://www.acquisition.gov/far and the Agriculture Acquisition Regulations - http://www.usda.gov/procurement/policy/agar.html. (iv) This solicitation is set-aside for Small Business under NAICS code 237310 Highway, Street, and Bridge Construction, and small business size standard is $33.0 million (v) Contract Line Item Number 1, Supply and Deliver 1(one) prepackaged 6 ft wide X 50 ft long Fiber Reinforced Polymer (FRP) Composite Truss Trail Bridge. (vi) The hike and mountain bike trail bridge is required for installation by USFS Trails Unlimited Enterprise Team See attached description/specifications and drawing. (vii) Deliver by April 7, 2014, to Caney Ranger District, Forest Service Work Center, 3288 Highway 79, Homer, Alabama 71040. FOB destination. All deliveries shall be made on regular workdays (Monday through Friday) during regular business hours (730am-3:00pm), excluding official Federal Government Holidays. Contractor shall notify Cam Lockwood at telephone (626) 233-4309 and James Williams, at (318)-927-2061, 24 hours prior to delivery for arrangements to open locked gate. (viii) The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Addenda to the provision; Submit Name, Address, DUNS Number, Point of Contact telephone and email address, Price, Response to Evaluation Criteria, and complete attached AGAR 452.209-70 Alt 1, as your Quote. (ix) The provision 52.212-2, Evaluation-Commercial Items, applies and the specific evaluation criteria is attached. (x) SAM (System for Award Management), is a Federal Government owned and operated free web site that consolidates the capabilities in CCR/FedReg, ORCA, and EPLS. Contractors are required to be registered in SAM with the applicable NAICS code, via https://www.acquisition.gov. For questions concerning registrations, certifications, or assistance with preparing offers, contact the Procurement Technical Assistance (PTAC) http://www.aptac-us.org/new/. An offeror shall complete only paragraph (b) of 52.212-3 provision if the offeror has completed the annual representations and certifications electronically in SAM. (xi) FAR Clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and addenda: FAR 52.204-7; 52.204-13; 52.225-9; 52.232-39 and AGAR 452.209-71. (xii) FAR Clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition, additional FAR clauses cited in the clause are applicable to the acquisition and will be provided upon award.
