Description

SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is conducting a market survey to identify potential sources to provide autopsy services. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources of providing autopsy services for the Lebanon VAMC. The services will include the transport of the body to and from the Medical Center to perform the autopsy service. Service must be available within eight (8) to twenty-four (24) hours of notification being provided of need. A written preliminary report is needed within twenty-four (24) hours of the autopsy being performed and a written Final Report must be provided within sixty (60) days The North American Industry Classification System Code (NAICS) is 621999 and the small business size standard is $14.0 million. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of autopsy services. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.Company Name, Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 621999 and the small business size standard is $14.0 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3.Provide a summary of experience related to autopsy services. 4.Provide a description of the ability to meet the services identified in this sources sought notice (i.e., the transport of the body to and from the Lebanon VAMC within 8-24 hours' notice; submission of preliminary and final written reports within the associated timeframes). 5.If company holds a Federal Supply Schedule (FSS) Contract, please list the GSA Contract Number and relevant SINS. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
