Description

SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is conducting a market survey to identify potential sources to provide part-time Histology Technologist services. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources of providing Histology Technologist services for the Lebanon VAMC at the Philadelphia VAMC location. The Histology Technologist will need to be available between the hours of 4:00 AM and 8:00 AM for requirement performance, and possess the necessary credentials as licensed by the College of American Pathologists (or equivalent). The provision of services is for the completion of workload as it is available on a part-time basis. The North American Industry Classification System Code (NAICS) is 621511 and the small business size standard is $30.0 million. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of Histology Technologist services. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.Company Name, Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 621511 and the small business size standard is $30.0 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3.Provide a summary of experience related to Histology Technologist Services. 4.Provide a description of the credentials your firm's Histology Technologists would or do possess. 5.Provide a statement of the ability to provide a Histology Technologist in the Philadelphia VA location for the part-time hours described in this announcement. 6.If company holds a Federal Supply Schedule (FSS) Contract, please list the GSA Contract Number and relevant SINS. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
