- SOURCES SOUGHT NOTICE: The Department of Veterans Affairs Medical Center Washington DC (DC VAMC) is seeking sources from contractors to assist with providing Assertive Community Case Management Services, to include but not limited to linking Veterans and their families who are homeless or at risk of becoming homeless. The appropriate services are based on needs assessment, housing assistance and obtaining self-sufficiency. The VA DC Health Care System (VA-DCHCS)'s mission is to "meet Veterans and their families where there are"; employing a housing first model to rapidly reduce veteran homelessness, particularly those veterans who are presented with serious mental illness, and substance use disorder. VA-DCHCS intends to utilized the services of the contractor to assist veterans and their families referred by the VA Medical Center or identified by the outreach efforts of the assertive community case management team (ACT). The DC VAMC anticipates issuing a solicitation in accordance with Federal Acquisition Regulations (FAR) Part 12 for this requirement on or about March 25, 2014, with a closing date for proposals on or about April 25, 2014. The solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments. Large and small businesses as well as non-profit organizations are welcome to respond in the event no Service Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses have the capacity to provide this service. The NAIC Code for this solicitation is 621420. The government anticipates making a single award of fixed price, indefinite delivery, indefinite quantity contract for this requirement. To be eligible for award, contractors must be registered in the Central Contractor Registration data base at www.ccr.gov. Registration is free and potential offerors are encouraged to visit the CCR website and register as soon as possible to avoid delays should they be selected for a contract award.
