Attachment 3- Security Notice to Prospective Offerors Attachment 2- Business Declaration Attachment 1- Past Experience History Elevator Modernization/Maintenance Multiple Award IDIQ The Federal Aviation Administration (FAA) is seeking competent and qualified elevator maintenance contractors interested in providing offers for a Full Service Elevator Maintenance Contract for approximately 8 locations. This is not a Screening Information Request (SIR) or Request for Proposal (RFP). See Section III for documentation required to be submitted and evaluated to determine which vendors receive the solicitation. SECTION I REQUIREMENT SCOPE The anticipated facilities that require the Full Service Elevator Maintenance are as follows: Omaha, NE (Eppley) Air Traffic Control Tower Lincoln, NE Air Traffic Control Tower Wichita, KS Air Traffic Control Tower Chesterfield, MO SUS Abilene, TX Air Traffic Control Tower St. Charles, MO TRACON Roswell, NM Air Traffic Control Tower Houston, TX IAH Air Traffic Control Tower Work to be performed includes but is not limited to: all steps and measures to provide a minimum elevator operational availability of 98% and maintenance of the elevator plant to maximize the life expectancy of the elevator plant in accordance with the latest edition of American Society of Mechanical Engineers (ASME) A17.1 -Safety Code for Elevators and Escalators. SECTION II- CONTRACT TYPE AND EVALUATION CRITERIA This will be awarded as a single award contract with a one year base period and two one year options, per location. This project is unrestricted, and will be solicited under full and open competition to those vendors who meet the criteria listed in Section III. The North American Industry Classification System (NAICS) is 238290, Other Building Equipment Contractors. The small business size standard is: $14 Million. Award will be made to the Lowest Priced Technically Acceptable (LPTA) offeror. SECTION III- GO/NO GO CRITERIA The initial information submitted will be evaluated on a Go/No-Go basis. Offerors must receive a Go on all of the following criteria to be considered a viable offeror and receive the solicitation. o Ability to perform work and provide on-going preventative maintenance on numerous types of elevators. o Five (5) years of experience performing elevator maintenance at a 98% operational availability at (5) different locations simultaneously. Projects must be similar in size and scope and must be FAA, or other Federal, State, Airport Authority, or commercial organizations, acting as the prime contractor. Indicate the operational availability, contracting agency/organization, contract number or project identifier, point of contact, email address, phone number, and size and scope on the Past Experience History document (Attachment #1). Offerors are required to submit the following documents to be determined if eligible to receive a copy of the solicitation: o Confirmation that service can be provided on numerous types of elevators at the applicable locations stated above. o Past Experience History Document (Attachment #1)- Limit 3 projects o Business Declaration (Attachment #2) o Document Security Notice to Prospective Offerors (Attachment #3) The information provided will also be used as part of the responsibility determination. Responses must be emailed to Russell.ctr.rogers@faa.gov, no later than 1:00 PM (Central Standard Time) 3/21/14. Please place "Attention: Elevator Maintenance Request for SIR" in the subject line of your email. Requests received after this date and time will not be honored. Failure to submit all required information will deem the request for solicitation as non-responsive. NOTE: Contractors must have active registration in System for Award Management (SAM), formerly the Central Contractor Registration (CCR,) before award can be made. Contractors can register at www.sam.gov.
