Description

SCOPE OF WORK PROJECT NO: 695-13-153 PROJECT NAME: 111 Upgrade 111M EP Lab 5CS I.General Description: Provide all labor, materials, tools, equipment, and supervision necessary to renovate 5th floor C Wing South in Building 111 to support new cardiac electrophysiology (EP) equipment and support services, and to bring EP lab up to Operating Room Standards. Construction to include rooms VP 5-17, VP 5-18, VP 5-40, 5C09, 5C10, 5C11, 5501, 5503, 5503A, 5503B, 5506, 5507, 5507A, 5509, 5512, 5515, 5515A, 5515B, 5528, 5528A, 5530, 5531, 5533, 5534, 5535, 5535A, 5535B, 5535C, 5540, 5542, 5544, 5546, 5547, 5549-EC, 5665 and corridors C0520E, C5C03, C5C04, C5C05, C5C06, & C501A. In addition provide same services for the renovation and relocation of all existing services i.e., offices, storage rooms, equipment rooms, etc. as required and planned under this project. Work includes the complete demolition, removal and replacement of all asbestos insulated piping/ductwork/flooring/ceiling, duct insulation, floor tile, etc., architectural, mechanical, equipment, furnishings, and electrical work. All design and installation will be in accordance with current VAMC diagnostic care and design guide.
