- SINGLE SOURCE (APPLICABLE TO PURCHASES UNDER SAT $3,000 - $150,000) 1. Contracting Activity: NCO 12, GREAT LAKES ACQUISITION CENTER, 115 SOUTH 84TH STREET, SUITE 101 MILWAUKEE, WI 53214 3.Authority for Action: X FAR 13.106-1(b), Soliciting from a Single Source 4.Describe the supply or service to be acquired and the total estimated dollar value: MRI scans of research subjects for Dr. Dane Cook's studies "Imaging Pain Modulation in Gulf War Veterans with Chronic Muscle Pain and "Impact of exercise training on pain and brain function in Gulf War Veterans." Total estimated value is $100,020.00 for one base year and one option year. 5.List the proposed contractor (name, contact, and phone number) and describe the unique knowledge or capabilities of the proposed contractor: The proposed contractor is the University of Wisconsin Madison Waisman Center - Laboratory for Brain Imaging and Behavior. The contact person is Michael Anderle, 608- 265-6250. Dr. Cook's research protocol requires the use of a 3 Tesla Magnetic Resonance Scanner. This type of scanner is generally only found at major research universities. The Waisman Center at the University of Wisconsin has the requisite equipment for this study. 6.Describe your market research efforts and efforts to ensure that offers are solicited from as many potential sources as is practicable: The research grant specifically requires the use of a 3-Tesla GE SIGNA MRI scanner and a mock scanner for subject training and behavioral testing. There are only two such scanners in the Madison area, the other is also located at the University of Wisconsin (Wisconsin Institutes for Medical Research) and the fee for the scans would be the same. The Medical College of Wisconsin also has one of these scanners, but it is located outside of the Madison area where the study is taking place. 7.State why you feel that the anticipated cost to the Government will be fair and reasonable: The Waisman Center has quoted $475.00 per MRI scan for this study. The costs for previous contracts range from $475.00 to $500.00 per MRI scan (see VA69D-12-P-3602, VA69D-12-C-0 159, and VA69D-12-C-0229). In accordance with FAR 13.106-3(a)(2) price reasonableness may be based on a comparison of the proposed price with prices found reasonable on previous purchases. The CO determines the proposed price for the current acquisition to be fair and reasonable. 8.Other: N/A
