- Prevailing Wage Rate Vendor company description This is a Subcontracting Opportunity with Exeter Job Corps Scope of Work ID#: EX 2014 -78851 To complete the following: Install epoxy floor in both 1 st floor and 2 nd floor laundry rooms in Building E Table of Contents I. Proposal Requirements Project: Remove current VCT tile, prep floor and install new epoxy floor (approximately 700 square feet) Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator : Adams and Associates, Inc Mailing address: Exeter Job Corps 162 Main Street Exeter, Rhode Island 02822 Proposal Due Date: 5 PM on 04/8/2014 Evaluation Criteria Point Value PRICE: Responsible Contractor who can perform effective services at the lowest reasonable price 60 QUALITY : Demonstrated experience and knowledge capabilities; Technical Excellence; Management Capabilities; Personnel qualification; Quality of the Tender packages; Technical excellence including where necessary, technical drawings, blue prints, 20 PAST PERFORMANCE : Past services to center or list of references for verification purposes 10 SCHEDULE : Detailed explanation on how services will be carried out and the delivery timeframe. 10 TOTAL POINTS 100 Payment: Payment Terms are Net 30 days of completion and receipt of Invoice. Site Visit Date: 03/25/2014 a) Contractor must visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with EX-2014-78851 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. II Project Description Scope of Project: Remove current VCT tile, prep floor and install new epoxy floor (approximately 700 square feet). III Specific Requirements: •· Remove existing vinyl tile •· Prep slab and score to accept epoxy(remove glue, score slab, clean slab) •· Pour and spread new epoxy floor and pitch to drains •· Contractor is responsible for all demolition, and any other unforeseen occurrences while installing, as well as disposal of all demolition debris. Warranties: •a) The contractor is to provide written warranty for the above statements at time of completion of job IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. (See the attached prevailing wage rate) Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet Rhode Island statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Exeter Job Corps Center as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for the work. The project shall be completed within 30 calendar days of the award of the contract. Written authorization is required to extend the 30 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. Time cards must be submitted for Contractor employees. Conditional Waiver and Release Upon Final Payment form must be signed before final payment. V Proposal Form The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID # and address all items in sections II & III, to be considered. Bids must to be valid for 90 days. The Contractor shall submit the following information with the proposal: A list of materials and product data sheets (MSDS) proposed for use References Copies of Insurance Certificates and applicable licenses. Completed vendor questionnaire and W9. All questions must be in writing and sent to the Purchasing Agent burgess.annette@jobcorps.org No Phone calls will be accepted.
