Description
Synopsis: This is a modification for VA243-14-I-10574. Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, Northport VA Medical Center is currently conducting a market survey for qualified firms capable of providing "Shuttle services". The vendor will be responsible for, but not limited to: providing Shuttle services for Bronx VAMC and Mount Sinai medical center. 1. Contractor's driver shall pick-up passengers from and to Mount Sinai and Bronx VAMC on a daily basis. Monday through Friday except for Federal Holidays Federal Holidays New Year's Day Martin Luther King Birthday Memorial Day President's Day Columbus Day Labor Day Christmas Day Thanksgiving Day Independence Day Veterans Day James J. Peters Medical Center 130 West Kingsbridge Rd Bronx, NY 10468 The Mount Sinai Hospital 1 Gustave L. Levy place New York, NY 10029 2. Contractor shall maintain a daily log indicating the number of passengers on each run, which shall be turned over to Police & Security personnel at the Bronx VAMC. The daily log from the previous day shall be provided at the first run in the morning to the Bronx VAMC. 3. Contractor shall transport as passengers; Medical students, residents and Fellows, and employees, who are performing duties at both Medical Centers. All passengers shall have an Identification Badge from either institution. Contractor's driver shall be required to request identification of all riders. 4. Contractor shall provide bus transportation to accommodate no less than forty six (46) seated passengers. These vehicles shall be equipped with heating and air conditioning. 5. Contractor shall provide communication link, radio or cellular, to security office at Mount Sinai Medical Center and Bronx VA Medical Center in case of any delay or failure of service. 6. Contractor shall provide training for drivers relative to schedule, mail service, communication links, sensitivity training, riding eligibility and I.D. Cards. 7. The contractor shall stop and let off passengers only at the designated locations at each institution. No other stops shall be permitted. No other passengers other that the institutions' residents, students, and employees shall be permitted to ride on the bus. The North American Industry Classification Systems (NAICS) is 485410. Responses to this notice must be submitted in writing via email at safa.shleiwet@va.gov and must be received not later than March 14, 2014 5:00pm EST. No telephone inquiries will be accepted. Interested firms should also add their information to the interested vendors list for this announcement at www.fbo.gov. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 485410. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov). Responses to this notice must be submitted in writing via email at safa.shleiwet@va.gov and received not later than March 14, 2014 5:00pm EST.
