Attachment 3 Pricing Sheet Attachment 2: Evaluation Criteria Attachment 1: Statement of Work FEMA is establishing a contract vehicle for procuring shelter items that are not part of FEMA's current inventory. Contractor(s) shall provide individual shelter items listed in paragraph 5.1 of Attachment 1: Statement of Work (SOW) on an a la carte basis, as requested by FEMA for rapid deployment under emergency conditions. FEMA will award a Firm Fixed Price, Indefinite delivery/Indefinite Quantity contract based on this Request for Proposal (RFP). Please see Attachment 1: Statement of Work for additional details on this requirement. SUBMISSION INSTRUCTIONS Each offeror is requested to submit an electronic copy and in PDF format of your firm's proposal. The electronic version shall be submitted to the Contract Specialists, Keith Harding at keith.harding@fema.dhs.gov. Electronic copies of your porposal are due no later than 12:00 pm Eastern Time, on March 26, 2014. LATE QUOTES WILL NOT BE ACCEPTED. Questions regarding RFP must be submitted in writing and sent via email to the attention of Keith Harding at keith.harding@fema.dhs.gov no later than 12:00 pm Eastern Time, on March 24, 2013. Questions received after that date/time may not be addressed. PROPOSAL REQUIREMENTS: All quote submissions must include the following information (preferably to be provided on the quote submission cover letter): 1. a) Tax Identification Number (TIN) b) Dun & Bradstreet Number (DUNS) c) North American Industrial Classification System (NAICS) Code: d) Standard Product Code e) Contact Name f) Contact Email address g) Contact telephone h) Complete business mailing address 2. a) Before award can be made offeror must be have an active registration with the System for Award Management www.sam.gov.
