- CLASSIFICATION CODE: Q - Medical Services NAICS CODE: 621 - Ambulatory Health Care Services; 621111 - Offices of Physicians (except Mental Health Specialists) Network-9 Contracting Office on the behalf of the Memphis VA Medical Center has developed this Request for Information (RFI)/Sources Sought Notice to solicit market research information from the healthcare industry to assist the Government with analysis of a Government requirement and provide commercial insight on performance-based solutions, best practices, possible contractual arrangements, and potential issues. THIS IS A REQUEST FOR INFORMATION ONLY. It is not a Request for Proposal, a Request for Quotation, an Invitation for Bids, a Solicitation, or an indication that Network-9 Contracting Office (NCO-9) will contract for the items contained in the RFI / Sources Sought Notice. This RFI/Sources Sought Notice is part of a Government market research effort to determine the scope of industry capabilities and interest, in accordance with Federal Acquisition Regulation (FAR) Part 15.201(e), response to this RFI / Sources Sought Notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI / Sources Sought Notice are strictly voluntary and are consider as information only. The Government will not pay respondents for information provided in response to this RFI / Sources Sought Notice. Responses to this RFI / Sources Sought Notice will not be returned and respondents will not be notified of the result of the review. If a solicitation is issued, it will be announced on the Federal Business Opportunities Website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from any response to this announcement. This RFI / Sources Sought Notice does not restrict the Government's acquisition on a future solicitation. Network-9 Contracting Office (NCO-9) and Memphis VA Medical Center is conducting market research for a Contractor to furnish board eligible and/or board certified Vascular Surgeons to provide vascular surgery care. The general scope is for a firm-fixed -price hourly rate for indefinite delivery indefinite quantity (IDIQ) contract for of the following services: "Contractor shall assign a physician to perform Compensation and Pension (C&P) examinations for vascular surgery patients requiring C&P examinations. Contractor shall ensure that physicians provide educational training sessions for vascular surgery section house staff, nurses, health techs, and vascular surgery physician assistants. "Contractor shall provide vascular surgeon(s) to support the Vascular Surgery Clinic Monday through Wednesday and Vascular Surgery Lab when the VA's vascular surgeon(s) are on leave. Vascular surgery clinic hours will be designated by the VA. The contracted vascular surgeon will be required to operate the clinic, examine patients, and interpret reports as required by the VA. The contractor's employees will be required to work with VA staff. Should additional clinic hours be needed to meet veterans' needs or the days themselves need to change, then all effort will be made to have mutually beneficial days and times. However, needed change to accommodate veterans' needs will be in place as early as possible but notification of change in clinic hours will be made no later than sixty (60) day advanced notice of requested change. "If contract surgeons are involved in resident education the surgical service chief shall designate a Resident Review Program Coordinator to evaluate house staff performance on the Vascular Surgery Sections. All problems are to be reported to the VA Chief, Surgical Service. "Contractor shall prepare monthly call schedules for coverage of vascular surgery emergencies. A draft schedule shall be submitted to the office of the VA Chief, Surgical Service/Administrative Officer within five (5) working days prior to the end of each month. "Contractor shall staff the Operating Room (OR) Thursday, Friday, and on call. OR days subject to change in concurrence with the Chief of Surgery and Chief of Anesthesia. Contractor shall coordinate with inpatient attending's to effect timely completion of medically necessary surgery. "Contractor shall ensure that contract physicians contribute to the ongoing quality improvement programs in the organization as described in Medical Center Memorandum No. 00-12, Quality Management System may be accessed at the following URL: http://vaww.memphis.va.gov/docs/memo.aspx. "In addition, the services to be performed by the Contractor will be done so in accordance with compliance with VA mandates, policies and procedures, The Joint Commission, Federal and State laws, and regulations of the Medical Staff by-laws of the VA Medical Center. All surgeons will be required to work in collaboration with surgeons who are part-time or full time salaried by the VA including but not limited to sharing call, seeing patients and operating when on call and collaborating physician's patients. In general the responsibilities for inpatient care include, but are not limited to: GENERAL PRIVILEGES: Vascular surgeries ward patients: "Admit "Diagnose "Develop treatment plan "Manage "Discharge "Including all needed medical documentation Surgical Intensive Care Unit patients: "Admit "Diagnose "Develop treatment plan "Manage "Discharge/transfer "Including all needed medical documentation Perform inpatient and outpatient consultation Provide clinical guidance and teach: "Senior house staff "Junior house staff "Intern "Medical students "Vascular Fellow SPECIAL VASCULAR PROCEDURES: "Abdominal aortic aneurysms " Aorto-bi-femoral bypass " Femoral-popliteal bypass " Femoral-femoral bypass " Axillary-femoral bypass " Groin reconstruction " Infected grafts " Pseudoaneurysms " Visceral arterial procedures " Hemo-Access " Thrombectomy " Amputation procedures of lower extremities " Carotid endarterectomy " Thrombo-embolectomies " Insulin pump " Varicose vein operations " Sympathectomy, dorsal " Aneurysmectomy " Embolectomy " Vascular grafts " Renal Shunt " Non-invasive vascular procedures " Fluoroscopic procedures performed in Operating Room* Endovascular procedures - AAA repair Lower and upper extremity intervention Visceral and mesenteric interventions "Dialysis access procedures Vascular surgery operations are to be performed either by the attending physician alone or if the contract surgeons are involved in resident education, the attending physician providing professional direction and supervision must be performed to the house staff at levels defined in the Veterans Healthcare Administration (VHA) Handbook, 1400.1, Resident Supervision Handbook, paragraph 7c(5)(c)1-6. VHA Handbook is available for viewing in the Department of Veterans Affairs Medical Center, Chief, and Surgical Service's Office or upon request from the Contracting Officer. If contract surgeons are involved in resident education professional supervision to the house staff, must be provided with documentation in compliance with VA mandate, regulations and policies. Pre-operative Note: This note can be written by the attending physician or by the resident, if the contract surgeons are involved in resident education. A resident note must reference attending surgeon's name, reflect his/her agreement with the procedure, and then be co-signed by the attending physician. Written Operative Note: In addition to the Operative dictation, a written operative must be documented in the Progress Notes. This note shall include: Date and Time Preoperative Diagnosis Postoperative Diagnosis Procedure Surgeons (must include attending) Anesthesia Administered Findings (including pertinent normal findings) Fluids (Both intake and output of all fluids and secretions given or taken from the patient. These include: colloids, blood, coagulation factors, NG, etc., and such things as urinary output.) Complications Condition of the patient upon leaving the operating room Any tubes or lines that were placed into the patient (i.e., drains, central lines, Swan-Ganz catheter, etc.). The contract surgeon must indicate his presence by signature. Operative Report: This report will be dictated by the surgeon immediately after the procedure. Operative reports are signed by the attending physician following transcription. Temporary suspension of the vascular surgery team's operating room privileges will occur when dictations are delinquent by twenty-four (24) hours. Results of fluoroscopic procedures performed in the operating room by vascular surgeons must be included in the operative report. Post-operative Note: If contract surgeons are involved with resident training, at a minimum, prior to discharge, the attending physician will evaluate the patient's response to the procedure and discuss with the patient the consequence or outcome of the procedure. Evidence of such a conversation shall be documented in a progress note, or countersignature of, or reflected within, the resident's note. Provide professional direction to support personnel employed by the VAMC, Memphis, TN. The Contractor shall provide Vascular Surgeons with vascular surgery expertise for peer review of selected cases. This shall include, but not be limited to: tort claims, patient family complaints, peer review, surgical case reviews, and benefit appeals. The Contractor shall be responsible for the administration and management of all aspects of this contract and the health care plan covered hereunder. This includes responsibility for all Contractor employees, subcontracts, agents, and any action for or on behalf of the Contractor. QUALIFICATIONS: Personnel assigned by the Contractor to perform the services covered by this contract shall have a full and unrestricted license in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and must undergo credentialing and privileges in accordance with the VA Credentialing and Privileges Committee and approved by the VA Medical Center Director. Personnel assigned by the Contractor to perform the services covered by this contract shall be proficient in written and spoken English (38 U.S.C. 7402). Contractor's assigned personnel must be computer literate as patient documentation is directly input in the patient's electronic medical record and must undergo training at the VA. ? WORK HOURS: Contractor's employee must be present at the VA facility and must be actually performing the required services for the period specified within the contract or the contract cost will be decreased accordingly, during each billing cycle. Need to meet timelines standards for clinic appointments, e.g. if they are on holiday, need to have "make up clinics." The contractor is responsible for planning and coverage during periods of leave. The payment for any leave, to include sick leave, annual leave, administrative leave, and holidays are to be the responsibility of the Contractor. The following terms have the following meanings: Work Hours: Scheduled Office Hours are Monday through Friday, between 8:00 a.m. to 4:30 p.m. (1)Operating Room: Monday through Friday. (2)Attending Duties: Hours are flexible depending on the needs of the patients.
