Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Combined synopsis/solicitation W90M8L40350102 is being issued as an unrestricted Request for Quotation. Northern America Industry Classification System Code (NAICS) is 621111. Specifications: The USPFO for Utah has an immediate requirement to contract for Respiratory Fit and Health Testing Services for the Utah Army National Guard at three designated locations. Tests are for approximately 130 participants and shall include the following: 1) Respiratory fit testing IAW CFR 1910.133. Contractor shall have the capabilities to fit test North 7500, 3M 7800 and Survive Air respirators. Contractor shall provide initial respiratory fit test and appropriate size for specified personnel. Tests shall be reviewed and approved by an OH board certified physician. 2) Vision testing, PFT, Height, Weight, Blood Pressure Testing, and OSHA questionair. Lead levels will be drawn on fueling personnel at the Army Aviation Support Facility (AASF). All will be IAW CFR 1910. Records and test results shall be reviewed with an OH board certified physician with notification given on any employees that require follow-up evaluations 3) Hearing exams IAW DOHRS-HC and input results in DOHRS-HC program. 4) Contractor is responsible to furnish all personnel and equipment necessary to provide testing. The Government will provide suitable space, electrical and water connections. 5) Contractor shall notify OHM of any problems indentified during surveillance Specific dates are to be determined; however, testing will end 31 August 2014. Testing locations are as follows: Utah Army National Guard Headquarters 12953 South Minuteman Drive Draper, UT 84020 Utah Army National Guard - Army Aviation Support Facility 7563 South Airport Road West Jordan, UT 84660 Utah Army National Guard - Manti Armory 85 West Union Street Manti, UT 84642 Evaluations Factors: The contract type for this procurement will be a firm-fixed price and award shall be made on the basis of best value. A quote with represents best value is one that conforms to the Government's requirements at a reasonable and competitive price. Quotes are due NLT 1700 hours (MST) on 28 March 2014. Quotes will be emailed to otha.b.henderson.mil@mail.mil. Facsimiles are not acceptable. All quotes must be received at the specified email by 1700 hours (MST) on 28 March 2014.
