Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6-streamlined procedures for commercial items, in conjunction with far subpart 13 and as supplemented with additional information included in this notice. This is a Total Small Business Set-Aside requirement only qualified offerors may submit bids. Solicitation VA-249-14-Q-0374 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. NAICS Code is 561710 and size standard is $7.0M. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. Wage determination WD 2005-2495 Revision 16 dated 06/19/2013 applies to this request and can be accessed at the below link: http://www.wdol.gov/wdo/scafiles/std In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov. Period of Performance (POP): Base Year - May 01, 2014 to September 30, 2014 Option 1 - October 01, 2014 to September 30, 2015 Option 2 - October 01, 2015 to September 30, 2016 Option 3 - October 01, 2016 to September 30, 2017 Option 4 - October 01, 2017 to September 30, 2018 Description of Services: This requirement is for the VAMC Memphis. The contractor must the Contractor shall provide all management, licenses, laboratory testing, tools, supplies, equipment, transportation and labor to support the Integrated Pest Management (IPM) plan of the VA Medical Center, Memphis, TN. The IPM plan shall address continuous monitoring, pest response and removal procedures, record keeping, warranties, pest education and communication to installation personnel to prevent pests and disease vectors. The IPM approach shall use targeted (effective, environmentally sound) methods, including check point, stealth fly, ground force ant, habitat/facility modification, biological/genetic/cultural control, mechanical/physical control, and, where necessary, the judicious use of least hazardous pesticides. The IPM plan shall comply will all applicable Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), state/local/federal regulatory guidance on pesticide storage, disposal and applications. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to the lowest price quote which conforms to the requirements within this solicitation. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include unit price, total, unit quantity, and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA documents in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable to shipping charges.
