Description

Solicitation number VA260-14-Q-0626 is being issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-72, dated January 1, 2014. The North American Industry Classification system code (NAICS) 541519 size standard of $25.5 million. This acquisition is set-aside for small businesses. All firms responding must be registered with the System for Award Management (SAM). Offerors quotes shall propose a firm fixed price for the services listed below, subject to the terms and conditions of this entire synopsis/solicitation. The location for performance is: Department of Veterans Affairs James E. VanZandt VA Medical Center 2907 Pleasant Valley Blvd Altoona, PA 16602 Quotes must be received by April 4, 2014 at 1:00 PM Eastern Time. Quotes must be in writing, and may be submitted by email, mail or in person. Preferred method is by email. Facsimiles will not be accepted. Description: The VA Medical Center is soliciting competitive offers for Planmeca Dimaxis to Romexis dental x-ray database conversion. The government intends to award a single firm-fixed price contract, without discussions, to the offeror who represents the best value to the government, based on the evaluation factors outlined under FAR 52.212-2 in the attached solicitation document. Quotes are requested to be FOB Destination, and inclusive of shipping charges. Line Items: LINE 0001: Description: Equipment, including all necessary hardware and software licenses for database conversion Qty: 1 LT LINE 0002: Description: Labor for design and implementation of database conversion, including all travel costs (if necessary) Qty: 1 JB Offers: Offerors must provide the following information in their quotes in order to be considered responsive: (1) The solicitation number; (2) The name, address, DUNS number, and telephone number of the offeror; (3) A technical description of the items being offered in sufficient detail to evaluate equivalency with the requested items in the solicitation. This may include product literature, or other documents, if necessary; (4) A capabilities statement outlining relevant experience and capabilities sufficient enough to determine whether the offeror is capable of performing the required work; (5) Terms of any express warranty; (6) Completed Price schedule and any discount terms; (7) A completed copy of the representations and certifications at FAR 52.212-3 (if not previously completed electronically in SAM); (8) Acknowledgment of Solicitation Amendments (if any); (9) A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. (10) Any other information required by this solicitation. By submitting an offer, the offeror agrees to hold the prices in its offer firm for at least 30 calendar days from the date specified for the receipt of offers, unless another time period is specified in an addendum to the solicitation. No phone calls will be accepted. Answers to questions will be posted as an amendment to this solicitation, if required. Individual responses are not promised, and should not be expected. Those incorporated by reference have the same force and effect as if they were given in full text.
