Description
The Department of Veteran Affairs, Dorn VA Medical Center, 6439 Garners Ferry Road, Columbia, SC 29209 has a requirement for DOSIMETRY EQUIPMENT. The Government expects to award to a single contractor a contract which shall be Firm Fixed Price with a firm quantity and no option years. This requirement is being set-aside for Small Business participation only. NAICS code for this requirement is 325992 with a small business size of 500 employees. Only offers from those firms regularly established in the business of providing this type of equipment will be considered. Subcontracting will not be allowed. All responsible sources may submit a proposal, which shall be considered. Please see solicitation for additional information regarding this requirement. PERFORMANCE BASED WORK STATEMENT DOSIMETERS - MONITORING OF RADIATION EXPOSURE 1. PURPOSE: The contractor shall provide monthly dosimeter for the monitoring of radiation exposure to personnel. This is a mandatory safety monitoring system to ensure a safe work-place environment, ensure equipment is operating appropriately (no radiation leaks with imaging equipment or laboratory devices). This enables the Columbia, South VAMC to be in compliance with federal regulations, The Joint Commission standards and Nuclear Regulatory Commission (NRC). 2. GENERAL: The Contractor shall provide labor, equipment, materials, transportation, necessary to provide dosimeters for personnel monitoring of radiation exposure at the Veterans Health Administration, William Jennings Bryan Dorn, Medical Center in Columbia and the Greenville Outpatient Clinic, South Carolina. 3. BACKGROUND: The WJBD VAMC is a 216-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. Annually, the medical center serves approximately 63,000 patients. Satellite outpatient clinics are located in Anderson, Greenville, Florence, Orangeburg, Rockhill, Sumter, and Spartanburg, South Carolina. 4. PERFORMANCE REQUIREMENTS: "M0101, 01 Film badge, approximately 135 badges per month "M0107, 19 MeasurRing (small, medium, large), 7 per month "M0184, TLD 760 with and with/out loop & clip, 8 per month "1953 billing code, Form 5, annual oindividual report, 2 ply 25 total "lost badges, badges that are lost by employee "expedited delivery, any badges sent emergently cost extra for delivery 5 YR "standard delivery of monitoring devices, monthly. Should be no charge "Environmental badges for quarterly monitoring, badge type is 17, quantity ois 12 per quarter if needed "monthly ALARA report on any individual exceeding regulatory limits "New badge set-up charge 60 YR *all numbers given are approximate and will be based on need during the contract. Orders for new badges and changes to the existing badges are to be made on a secure web page on the vendor's web site. Access will be made with a unique username and password for security. The site will permit adding individuals and badges for monitoring of radiation exposure. It will also permit removal of monitoring (badges) for individuals. The addition of an individual will include the person's name and full social security number. When an individual does not have a social security number, a passport number, or work permit number may be used. 7. SPECIAL REQUIREMENT: a. Member of the National Voluntary Laboratory Accreditation Program (NVLAP) sponsored by the National Institute of Standards and Technology. b. Does require internet management of service, thus the VA will provide sensitive information over the internet. 8. CONTRACT PERFORMANCE MONITORING: The government reserves the right to monitor services in accordance with Performance Based Matrix. 9. INVOICES: Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer's Technical Representative (COTR), and submitted to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714. A properly prepared invoice will contain: oInvoice Number and Date oContractor's Name and Address oAccurate Purchase Order Number oSupply or Service provided oTotal amount due
